I am going in to my PCP next Monday and I am going to be requesting the referral for the bariatric program hoping to be accepted for it - I am leaning toward the sleeve but what are the criteria they go by - I am 245lbs - 5'6" and my BMI is at 39 - does anyone know what the BMI requirements are?

Some insurance require 40 BMI or more. 35 if you have co morbidities. Like diabetes, hypertension. My insurance would only accept the diagnosis of morbidly obese (I weight 277# and am 5'6" )

Call your insurance co and ask them what their requirements and then tell your doctor what diagnosis you need. Will save you 30 days every time it is submitted.
