I have completed my 3 consecutive months of being medically supervised by a weight loss doctor. Can anyone tell me exactly what the office needs to send or write up for the surgeon.

They should already know that information. Its a letter of medical necessity.

Your Dr.'s office should have the guidelines based on your insurance policy. Most require more than just 3 supervised visits/months. You can call your insurance to find out what is needed, but your Dr.'s office should have it taken care of.

Thanks utcaneuser & sanction for the info. I told the weight loss office that I needed a letter of medical neccessity but the lady was asking what was included. I thought the office should know that part!!!!
