    Thank you so much to all of you for your replies. My consult is next week. I hope Dr. approves me for bypass as that is my primary preference but I know many Dr's push for sleeve nowadays. How about eating high fat low carb? Since high cat curbs your hunger and keeps u feeling fuller longer, can any of you eat traditional keto now years later? Or you still have to keep the fats low?     Gastric Sleevr Vs. Bypass

    I didn’t start at as high a bmi as you but I still thought I’d share my experience so far. I chose sleeve with my surgeon’s support, even though I suffered from stress related gerd. Yes, I have some issues but manage it with medication. I chose sleeve surgery because it doesn’t change the digestive tract as much as other surgeries. Yes my stomach is smaller but it is the same length as it was as are my intestines. I think this is why absorption is not as great an issue with sleeve. My nutrient absorption is good. I haven’t taken multi vitamins since I reached my goal a year ago & my blood work come back fine. Yes, bypass patients do tend to lose weight more quickly to begin but the 5 year average of weight lost & maintained across both surgeries is about the same - 60-65% of the weight initially to be lost. Dr Vuong has a great video that explains this very well. Think it’s called how to maximise your weight loss. The amount of weight you successfully lose & maintain really comes down to the individual. Even though I’ve exceeded my original goal (lost about 135%), I’ve had the view that I would find a place of balance between maintaining my weight & maintaining my lifestyle. I believe that if you restrict your food choices & intake to a point you can not enjoy your life & the things you like to do your diet will fail because you will consciously or subconsciously revolt against the restrictions you employed. Honestly, my diet is still pretty restrictive but I’m not quite 2 years out so I have a long way to go to find the balance of what works for me. Currently, I eat low sugar & artificial sweeteners, low fat, moderate carbs (whole or multi grains but no breads, rice, pasta), fruits, vegetables & of course I’m protein & portion control focussed. I have the odd glass of alcohol but I often don’t finish the glass. It’s easy to manage & doesn’t affect how I socialise so far. The keto debate is an interesting one. My 3 week pre surgery diet was keto but I know I could not maintain the high fat requirement long term but that is me - I don’t like overly fatty foods. But it certainly gave me a kick start. There is research that says the high fat content causes cholesterol problems. There’s research that shows that while it may help people with diabetes to begin, long term it can cause erratic insulin levels due to the low carb intake. And of course there’s research that it’s great. I think you have to find the diet that works for you. Good luck whichever surgery you choose.
  4. Try an electrolyte powder in your water. It is common for the liquid diets to trigger "keto flu" from the lack of minerals and nutrients compared to a normal diet. I found a packet of electrolytes a day really helped keep my energy up and the muscle cramps and headaches at bay. Good luck! ❤️

    Any sleevers doing a ketogenic diet?

    Hi BigUtahMan, The keyto diet is something I was really messing around with a couple of months ago. I agree, it helped me with hunger as well. I bought books, keytone sticks, read articles and watched youtube videos to gain a better understanding. I asked my nut about it and she just said not to worry about whether or not I was in ketosis, just to continue eating protein first and focus on whole foods...all the things she had taught me over the past year plus. I was also reading conflicting information, which is pretty much the case with anything you look up on the internet these days. It was a little strict for me. It was hard for me to process that if I ate one apple (20g of carbs) that I was probably over my carb limit. My nut is a fan of the paleo diet and it seems to be less restrictive, so I'm currently learning more about paleo. I think Keto can be very beneficial, but very stict. At 14 months out I find myself still learning.

    Update: I emailed a few articles about this to my sister (who is a MD) and she said it sounded bogus and backed it up with some good explanation/examples... I definitely have the rash too, but it seems like maybe there's a reason besides keto type diet behind it...
  9. I'm a 34 year old woman with a current weight of 260 height is 5'6. I have lost and gained over 60lbs at least 3 times. I was never able to maintain the weightloss for longer than 6-8 months at a time. I have done strict Keto, Fasting, and intense exercise routines multiple times in my life. I was successful in losing weight every time so it wasn't that I couldn't commit myself or apply weightloss techniques that work for me. I would just simply go back to overeating every time. I really don't know if fasting will work for me longterm if I add coaching. I certainly want it to work but I have small children and a husband who likes to eat big hearty meals so cooking and shopping for food are part of my regular daily activities. I find when I'm fasting it is really difficult to cook a hot delicious meal that I simply cannot even taste. So I end up cooking a quick meal for my family which is not the healthiest. I will keep exploring the surgery option. I really appreciate all of the feedback so far. The new ideas like searching for my own bariatric therapist and using my family doctor to do my bloodwork after surgery has given me some hope that I can do this. 😊

    Breads Products!

    Since someone asked, I eat double fiber wheat bread, which 4 grams per slice. So there CAN be somewhat high-fiber bread. Not exactly sure if/when/how much I'll be eating of that after surgery, though, and certainly during the losing phase I won't make it a staple or priority. My surgeon's dietitian recommends a normal healthy diet with (obviously) a focus on protein. I know a lot of surgeons recommend a keto-like diet and I'm very thankful mine does not. I don't see a reason why healthy foods like beans, fruit, and whole grains have to be cut out completely and put in the mental "bad!" category as if they're the same as a candy bar.

    Carbs!

    Right now? 30-50g a day because I'm on the liquid diet, lol. Post-surgery, long-term, I'll probably be at 65-70% of my diet as carbs because I'm a vegan. (Those carbs will be from fruit, veggies, beans, oatmeal, potatoes, bread, rice, and other whole grains, and very few if any from sweeteners or processed food.) That's way more than most post-WLS patients, who tend to severely restrict carbs, so this might not be the answer you're looking for. I recommend reading How Not to Die by Dr. Michael Greger or watching Forks Over Knives on Netflix for some info on how plant foods like I mentioned above help you maintain a healthy weight and protect you from disease. You can also check out Dr. Matthew Weiner on YouTube, who's a bariatric surgeon who promotes plant foods ahead of the standard protein diet. Just thought I'd share my opinion in addition to the many keto and low-carb comments you'll see on this thread. *makes angel face*

    Advice Needed

    I did two weeks on keto. Had one friend on shakes & another on a combination for three weeks. Seems it depends on your surgeon & your starting weight. Post surgery I was fluids for two weeks, purées for two weeks followed by soft foods for another two before beginning solid foods. Take it slowly. Your tummy will tell you what you can or can’t tolerate but I wouldn’t push it through the stages quickly. Took me a good three weeks to feel up to working but I always take a long time to get over things (healing, viruses, etc. - I have Gilbert’s & my BP was low). No lifting for two weeks & then less than 5kg for another two. Then no more than 10kg for the next two. Remember you’re healing from major stomach surgery. Always had a large glass of wine every night & enjoyed a good scotch or G&T. Now hardly ever drink. Lost the taste for it & don’t miss it. Certainly don’t need it to socialise with friends. Plus it was taking an hour to drink a glass. Would avoid mixing with juices - high calories & sugar. Also beware of addiction swapping. It’s a concern for post weight loss surgery patients to turn their addiction for food to something else. Carbonated drinks are very filling. Better to be getting in your protein & good nutrients than filling your small tummy with soft drinks. (Plus I personally have an issue with artificial sweeteners - they still feed your sugar craving.) If the tool is to help you & you intend to be successful long term you will have to change your relationship with food: how you eat, what you eat, when you eat. It takes work and your life will change but for the better.
  14. I'm not telling friends and family just yet. I know my family would argue with me about it: that it's a waste of money, too dangerous, etc., and that I should just do it "naturally." My surgery is scheduled for January 11th; I won't see family until sometime in May or June, so... I'll tell them then. And then they can yell at me all they want. (Family: Mom and Dad) I'll tell my boss and my immediate team members at work - mostly because I'm going back to work just a week after being sleeved and don't want them to think I'm just slacking off. At the moment, most folks, both friends, co-workers and family, know that I'm going to Thailand - I've had a vacation to Thailand booked for three years and finally scheduled it. But I'm too fat to go there right now; I've been dreading the trip and finally canceled it. Instead, I'm taking the time off work to go have the surgery. I don't want to lie to my boss and team members about where I'm going. Once my weight loss starts showing and if folks ask, I'll either tell them that I'm on a permanent "Whole30"-style diet (paleo/keto) or flippantly say something like: "Oh, yeah, I had half my stomach removed," depending upon who is asking. But I'm not openly telling anyone and everyone. Of course, I do have an acquaintance/casual friend who used to be quite heavy and very large. She started dropping weight rapidly and would tell us that she was just sticking to a very strict diet. It was only after about six months of this that, finally, my sister-in-law said to me: "Doesn't she realize that we all know she had some kind of bariatric surgery? I mean, we're not stupid!" hah! Um... till she said that, I totally believed our friend's story that she was dieting/exercising it off. Guess I'm the dummy. lol.
  15. Yes, it's not a surgery thing per se, but you get either what's known as "keto breath" when you are are on low calories diets where it smells like acetone or some variation on an unpleasant breath. Those dissolving breath tabs that you put on your tongue work great to fight them.
  16. I eat things like tacos with the shells made from chicken, cauliflower crust pizza, etc... Absolutely LOVE it. I buy a brand called Real Good Foods. Designed for people on keto and low carb diets, but also perfect for bariatric diets. I've never been brave enough to try to make a homemade pizza, but I might just look into this one

    Nice. Feels good to know I'm waiting on my spa day Also a huge +1 to everyone who has shouted out Dr. Weiner's videos. His videos are the first resource that made me feel I could actually permanently change my eating habits and actually enjoy it and gain the great health that comes with eating a diet largely based on plants. For a long time I assumed I'd have to be keto after surgery and I have never enjoyed that type of eating. In addition to all that, he breaks everything down to the nitty gritty, science-wise, so you can really understand and soak up the information. Those videos are wonderful!

    Keto diet

    For one example... My brother-in-law has been doing the keto diet mostly for over a year (did not have bariatric surgery). He lost 100lbs on it and is keeping it off. Also he used to be on cholesterol lowering drugs but he now has his cholesterol in check and doesn't need them anymore. His BP is also great. I can't figure out why so many people bash this diet. Just because it goes against the thinking that we've all been taught (low fat) for years doesn't mean it's wrong. When he talk to his doctor about it, his doctor told him just keep doing whatever you're doing because it's working great for you. I don't know how us "sleevers" would do on it since we now have a modified stomach, but if in a years time if I don't hit my goal weight with the diet my dietitian recommended, I'm going to try it for sure.

    My Hospital Bag..??

    I'll be bringing my laptop and laptop charger.and my cell phone USB charger. Good call on whomever said to bring a pillow to hold against stomach. Lots of us on here too are also July-ers......lol, right now I'm trying to survive the pre-op diet..I know it's not that bad, but I went from Keto (which I knew I couldn't do after surgery) to now this soft food, tomorrow start on full liquids...and my body is like...what the heck - you usually eat enough protein to keep us feeling ok... My stomach is wondering what the heck happened...well, I have NEWS for my stomach! It's going to be...um....er..not having to worry about most of itself after 7/1. Shhhh!! Right now I'm so dang HUNGRY!

    Yeah getting that water is hard, kind of like a full time job! But I will help! The reason I asked about the all protein thing. Was because people that eat a very high protein diet with very little carbs will fall into ketosis. Atkins like diets can cause "keto-breath," its caused when excess ketones in the bloodstream. one chemical component of ketones, the odor is accompanied by a foul taste in the mouth. When the body is in ketosis, urine may also take on a peculiar smell. The body's capacity to store glucose is limited, so when its reserve has been exhausted by fasting, skipping meals or strenuous exercise, it switches into ketosis mode even when you aren't following a ketogenic diet. The presence of ketones in sweat may give it an acrid, ammonia-like odor. Upping your carbs a little bit might help.

    Help with carbs.

    My understanding was that the "Net Carb" count was more for Keto / Atkins carb counting to be a guide for staying in Ketosis. Then it became a way for marketing to say its lower carb to sell more stuff. Both my primary care doctor and my surgeon told me to measure total carbs and disregard net carbs if I'm not doing either of those diets.

    Naughty days?

    I think this is great, but I also think that you're in the minority for those of us that are fat or were fat. I think it's also interesting that some people who CAN avoid 'trigger' foods for life seem to assume that it's that way for everyone. This assumption leads to a lot of negativity on forums and FB pages (at least from what I've seen) I'm not criticizing you or trying to get contentious - it's just what I notice. I joined this FB page for sleeve/bypass people to (supposedly) share recipes and stuff like that. The lady that ran it (also conveniently had a web store to sell shakes and stuff on) was 10 years out (bypass) and still lived a <20 carb keto lifestyle, maintained a 200lb weight loss, etc. I think the admins were all friends of hers, too. Holy **** was that a sanctimonious crew - one would think that it was some kind of religion where anything with carbs was essentially satan himself. The admins and the people who wanted to be like the admins would totally jump on people's **** when they admitted to having a cookie, or even suggested substituting something in a shake or recipe - in one post, some lady got reamed for saying she put half a banana in her daily shake. I left the group after a couple of days. Point being - some things will change for some people, but the same thing won't change for all of the people. I think we should all take great care to qualify any advice we give on nutrition and eating here - make it very clear that what you're talking about works for you, maybe not the OP. Oh, and be nice and supportive too.

    Liver Shrinkage Diet struggles

    Apparently they called the first week of the pre surgery shake diet hell week at my surgeon’s office. There’s a lot of withdrawal symptoms associated with it (sugar, carbs, caffeine). Your cravings go crazy - you always want more the things you can’t have. Your emotions are all over the place which often makes you want to comfort yourself with food. On & on. Round & round. I was vey relieved I was placed on keto for the pre surgery diet. I had done the shake diet before. Two shakes & one small portion meal of steamed vegetables & plain cooked protein. Did it for 8 weeks & then again for 6 weeks a couple of years later as another diet I was trying. It ain’t easy even with one meal. Check with your dietician regarding swapping out a shake for a broth/soup. Some companies do offer them as well as shakes but they’re still going to be low calorie & go through you quickly being liquids. You’ve got this. You can do it.
