ejm, did you have a DS done? I have read that in rare circumstances low protein levels or excessive weight loss can cause the small intestinal portion of the DS to have to be reversed. Have you spoken to your surgeon about the extra weight loss? Have you had your vitamin levels checked lately to make sure you aren't deficient in anything? It might be worth an appointment with the surgeon to talk about your concerns... I wouldn't cut out fat so much as just make sure your fat is coming from a heart healthy source. DS patients have malabsorption of fats which is part of what leads to their better weight loss/maintenance outcomes. But some DS patients find they need to eat more fat or calories to maintain a healthy weight, and things like nuts and avocadoes can help with that. sweetsmith78, I'm sorry you felt scrutinized. I have generally found people on this site to be quite kind and helpful. It is hard to know what to say to someone experiencing excess weight loss when most of us here don't have that issue or anything even close to it. Which procedure did you have? Have you talked to your surgeon about the extreme weight loss? I would think focusing on calorie dense food might help. Full fat yogurt, milk, and cheese. Maybe adding MCT oil to your coffee? Protein shakes with peanut butter added, in fact, anything with nut butters added. And adding more nuts and avocadoes to your diet, as they are good sources of healthy fats. For most of us fat is where we start to gain calories back (or sugar, but I generally don't tell people to eat sugar LOL). I'm sorry you are losing so much weight, it must be very distressing after having surgery to treat the opposite issue! My sister sits in the 90s/100lb range and when under stress she really fights to keep her weight up. She hasn't had any surgery, she was just born that way. She uses meal replacement shakes and fats to help with that. Good luck to you both with finding answers and viable solutions to this!

I hope you get answers . I'm having the same issue. I'm down to 90 pounds. It's not healthy or healthy looking. The only thing I can suggest that we do is watch calorie intake. But then again at this state. I regret doing what I did. Before I have mentioned this it seems as if I was scrutininized . I Just want support. Good luck ejm hope we get some thoughts on this

At first I was a calorie counter about 6 months. Now I just watched what I eat. That being said I stay away from only man made carbs and small portions. Unfortunately I need to start counting loosing to much🥴 -

Haven't been posting pictures here cause I get in a "rut" and tend to eat the same thing over and over until I can't eat it again for months lol. Here is my stir fry Bulgogi with konjac noodles (aka shirataki noodles aka miracle noodles), Chinese sausage, napa cabbage. Total calories 185. I didn't finish it all but I counted it all anyway because I will probably finish it later today. -

I'm taking stock of everything I do, and don't do, every day to see what might need to be tweaked a bit. I already write what I eat everyday. I track my calories, carbs, protein, and fat. My doctor asked me if I wanted to try Metformin for a bit to see if that helps, and if it doesn't, I could try one of those weight loss shots. But I really don't want to do that. I know I can get my body to cooperate, I just have to figure it out. If I need the shot to lose the weight, my body will depend on it, and once I stop, the weight will come roaring back. I know there's a way to get this last 50 pounds off, I just have to figure it out. -

You have had the roughest ride of all of us. Your body is saying enough, I need a holiday. Let it go to the beach. Eat well, exercise to a limit that will not tax your body. Kick back and look at how far you have come I am a great believer of being relaxed about the whole new lifestyle. I am not on a diet. What I do now, I will be able to continue with for the rest of my life. I don't push anything. I don't change the way I do anything, I have watched people do extreme diet and exercise and to be honest it has never worked for them. They fall off the regime that's too much to hard to stick too. I just stick to my teams advice. At the moment they want me to up my calcium and keep below 1500 cals. I don't lose for months and then I clunk down 5 lbs a time. Then the long stall starts again. On here I was advised that a 3 month stall meant that I was doing stuff wrong. The person who said this had not had the surgery and not walked in our shoes. I didn't take offence but oh boy is that person going to have a shock when they get into a stall. Its not all about a calorie deficit. -

That cheesecake @GreenTealael looks amazing. Yum! I round up the calories & down the protein. I also call fluids in soup, etc. & what I drink to swallow my meds bonus fluids & don’t count them either. Lol! My oven isn’t working properly. Waaah! Interestingly it worked fine on a Sunday then three days later after the electrical company turned off our power for 8 hrs it didn’t. Hhmmm?? The top element heats. The bottom element heats but turn on the fan & nothing, no heat. Have to go back to old school non fan baking or call the service team to come out. Gracious knows how much the call out alone will cost. Service call-outs have got ridiculously expensive. Are they crazy in the US too? -

I would never have believed I’d ever be able to eat 1500 calories and not only maintain my weight but also not gain. I’d gain weight eating 1200 calories before surgery. It took me a good 2 months to barely lose 4kgs (about 8lbs) eating less than 500 calories with the last diet I tried! The boost to your metabolism the surgery gives you is amazing. You’ll have a new norm. How much of a boost is unknown. But the calculator will give you an idea of how much you may be able to consume - it’s still just statistics based on a narrow range of factors. Then it just depends on your body/metabolism, medications, genetics, etc. Use it as a guide & see how you go in practice. -

Oh that BMR calculator made me laugh... It estimates around 2,000 calories at my current weight and about 1,600 at my goal weight! Both are actually higher than my norm. I average around 1,200 to 1,400 calories a day (yes, tracking EVERYTHING, even drinks and junk). I've averaged that for the last 10+ years and still managed to gain about 100 lbs. But medications and health conditions helped with that gain... Still, if I actually end up at around 1,600 a day with no exercise and no gains I'll be one happy bunny! I guess it really is a matter of perspective and what we are used to. I've always thought the 2,000 calories a day thing was a load of garbage. Caloric needs vary widely depending on body composition, activity level, health issues, etc... I'd hate to see what weight I'd be at if I was actually eating those 2,000 calories it thinks I need right now. -

The only area of caution with that is to get to that calorie number, it's got sort of a lot of sugar for people like us, and if OP is susceptible to dumping syndrome, they will need to either break it up into multiple, manageable servings or choose something less glycemic (the only reason I say this is that I have been in that situation myself, and those Ensure shakes made me dump if I drank the whole thing in one sitting. I had to break them up into 3 "parts" and would drink 1/3 30 min after each meal) -

Already drinking a couple protein shakes a day. Haven’t been able to get that many calories in yet. The most I’ve hit yet has been 800 and I felt horrendous. Just bloated. But I am working on every suggestion to see what seems to work, so thank you. -

I’m getting to the 1 year post operative point and just to slow my weight loss my minimum calorie intake was set to 2300 calories, very hard to do and follow my desire to only eat whole non processed food, so they have asked me to stop exercising until I can get 2300 a day and stay at a steady weight. It’s actually really hard to consume that whole foods to hit that 2300 calorie goal. Anyone else have similar problems? -

What about adding a higher calorie shake like the Ensure Complete Nutrition, which has 350 calories in a 10 oz serving? It's generally considered too high in calories for using if you're trying to lose weight, but if you had three of those per day, you'd be getting 1050 calories and 90g protein right there before even having soft foods. You should be able to manage that amount capacity-wise with no problem. They have 8g fat and 15g added sugar, which might cause some risk of dumping, but I would think it would be worth a try. It seems like you'll need to hit about 1400 calories per day minimum to stop losing weight, whereas in the early days the guidelines you get from the doctor that are intended for weight loss may put you well under 1000. -

At almost 2 years out I can almost eat anything. Some meals are always the same. Breakfast is always Greek yogurt and berries. My lunch is usually a bit like pick and mix - a bit of a few things, Which could be a selection of these, Smoked salmon, deli meats, burrata, cashew nuts, walnuts, blue stinky cheese, any fruit, tuna, boiled egg or any left over from a previous meal. The evening meal is usually food cooked from scratch for the family with carbs that I can not fit in my stomach yet, casseroles, roasts, pies, stews. I eat what I can. Later in the day I either eat my meals, left overs or have a protein bar or a protein yogurt. My protein is about 80 grams a day now and my calories are between 1200 and 1500 -

In the early stages after bypass, there is a higher risk of dumping. I think that's why we're told to steer clear of the carby foods (aside from the weight loss aspect of things). I'm with @Arabesque, it's important that you consume highly-nutritious foods right now, while you're restricted. Perhaps adding fat will help? Again, with the risk of dumping, there may be some trial and error to work through. But fat is calorie-dense. Can you tolerate protein shakes? Adding a few of those each day might be a good way to boost your calories. Also, excess protein is stored as fat - that might be helpful for you. -

Transitioning to maintenance was both easy & challenging for me. Easy was that I had already made my choices about how/what I was going to eat & had been eating that way for most of my weight loss. What I found difficult was being able to eat portions large enough to stop the weight loss & work out how much I needed to consume to maintain. It’s why I kept losing for another 11 months. Though I also believe this was where my body wanted to be. Yes, I weigh myself regularly & yes I worry about what I weigh right down to the grams. While I don’t consistently weigh & track what I eat every meal I am conscious about portion sizes & the calorie content of foods even when I’m dining out or socialising at a friend’s. I do random checks & introduce new foods/recipes slowly & carefully. And yes, I still do protein first, then vegetables too & aim to stick to nutrient dense foods. This is the longest I’ve been able to maintain my weight ever in my 58 years. But I’m not on a diet to lose or maintain my weight. This is just how & what I eat. (The power of words.) I enjoy eating & food but it’s a different focus in my life - it’s more about what I need & less about what I want. Hell yeah, the old mindless eating habits are still just there sitting on my shoulder. (Can’t eradicate them though would be amazing if we could.) A bit like that angel on one shoulder & devil on the other. I try to keep the Angel of new habits I’ve established as the dominant thinking & actions. On the days the devil is stronger, I usually catch myself but … there are times. Though I try not to beat myself up if the devil wins. -

I’d worry about filling your tummy with a nutrient poor carb (pasta, rice, bread) especially at this stage when you’re on such a restricted diet still, eating so little & not being able to get in the more important nutrients. Remember protein, protein, protein. I presume you are still drinking protein shakes to get in the protein you need. What about making your own protein smoothies with more nutrient dense ingredients - like vegetables (blend & strain). What about adding a soft cheese to some meals or as a snack or some good fats like avocado. Add milk powder to cream soups, smoothies, etc. for extra protein & calories too. What about custard - calories & protein. You’ll likely find that once you’re through the first few months & the restricted diet, & able to eat more without vomiting & the other gerd related issues restricting your ability to eat & consume calories, you will put on weight & attain a weight you’re happier at. What does your surgeon say? -

I started off tracking everything in My Fitness Pal, but it got to be too much. Now, I keep an informal daily calorie tally in my head, and so far it's working. I also weigh every morning without fail to make sure I keep things in check. I know some people advise against it, but it works for me. -

Well I guess I will go first and see if the July Peeps will update. As of Today I’m down 45 lbs and have 37 lbs to go. Lately I stalled for about a week at 223 lbs, but got through that last night with an insane workout day. Seems harder and harder to lose a single pound. Yesterday burned 4700 calories and ate around 800-1000 calories. Recovery went extremely fast for me thankfully. Hopefully 🤞 I won’t need yesterdays effort to continue to lose…. Lol 😂 -

For me that's been a razor-thin line because my calorie numbers are so small to begin with (I am less than 5' tall)...1400 was too many, 1100 is too few, and it varies wildly depending on how active I am (I ride a motorcycle and it burns 200 calories an hour...when I'm on a trip I have to go out of my way to eat a LOT extra to not come back from a weekend trip with all my clothes too big). I try to stay between 1200-1300 and that's working OK, except for when I can't hold that much, which happens sometimes. -
Starbucks Coffee or Latte's

I would seek out an independent coffee shop over Starbucks; a lot of the little mom and pop coffee places near me have a much better selection of sugar free syrups and toppings. One thing about the Torani white chocolate syrup vs the Starbucks one is there is a pretty big difference in texture between the two; what Starbucks uses is a thick, viscous syrup that's almost the consistence of a sundae syrup or honey. The Torani product is clear in color and has a consistency similar to water. It tastes really good, but it may make your drink feel like it has less "body", and I have a trick for that. Put a scoop of collagen in your coffee. You won't really be able to taste it but it will give it a richer mouthfeel and you'll get a nice skin-enhancing boost from it for 40 calories, much less than adding that thick Starbucks syrup. -

I have trouble with a lot of pasta, rice, potatoes and bread still because they always feel like they "swell" in my stomach, so gaining weight was hard for me after I too found myself underweight following a health scare. I'm also a sleeve patient who dumps, so eating things like ice cream was out of the question. If you are ABLE to eat those things without issue, I don't see there being anything wrong with consuming a little extra treat here and there to gain weight, unless you are concerned that once you get back to a weight you're happy at, you won't be able to stop eating those things. I was eating peanut butter quite a bit when I was trying to gain; I found some high protein waffles that agree with me (Kodiak), so I'd eat one of those with peanut or almond butter on it, maybe a LITTLE drizzle of honey, a few dark chocolate chips, or some sliced banana. It was giving me more calories with a nice healthy dose of fats and protein vs carbs, which felt "healthier" to me.
