At 287lbs and 49BMI, I started trying to get serious about weight loss and weight loss surgery on my own with only my PCP on board. My oncologist had been trying to talk me into WLS for 2 years and I resisted his offers of referral to a program. I wasn't ready to change. So it took 2 years to get my head in the game! And when I did, I didn't look back--even though the fear of affording the post WLS life gripped me! My first visit with the surgeon, I weighed 256 and was just over 41BMI-ish. She required I lose 10% of my total body weight in the 6 months leading up to the pre-op liquid diet and I was so fearful of not being able to comply. Cuz if I could lose on my own, why would I need WLS? But WLS doesn't just help us LOSE the weight, it helps KEEP IT OFF longterm. I had to pay my nutritionist for each of 6 monthly visits out of pocket. It was so worth it and worth cutting corners elsewhere to help. I can still see her whenever I need to. BUT, she runs a weight loss support group where I get to see her for free once a month at a hospital. It's a great group of ladies who meet to talk and offer support. And there is no cost. Most hospitals offer free support groups and allow outsiders (people from other surgeons) to attend, cuz you never know when someone is gonna need a revision. So look up all the WLS practices in the area--especially ones centered around large teaching hospitals, and see if you can find one! I didn't go to a gym or work with an exercise physiologist--no funds to do so--but I was prescribed PT a few times for pain and rehabilitation. So see if you can start there and get that covered. Throughout the losses, I have walked daily and do sporadic core strengthening exercises at home using bands and isometrics. I now weigh 135.2lbs today. -
Well I had Chronic gastritis, GERD, documentation of a Gastric Ulcer, who I had christened Hector, since his pain hectored my existance quite a bit, and I marched right up for an RNY bypass, never had a VSG or less considered. Remember my surgeon asking if I needed advice to my choice? Nope knew with a 1st cousin dying of esophageal cancer, starting to show Barretts MYSELF, GOD and I had made our decision, he gave a slight chuckle and said then the 3 of us concurred because that also would have been his professional advice. And frankly I had reached the point that surgery was much less frightening than continuing without intervention. And even with my Rocky twisting road of Recovery from it all, I NEVER WOULD want to go back to BEFORE when the HEREAFTER is So Much Better.

I had the on-Q when I had thoracic surgery to remove a tumor from my esophagus. It is a little tube that is inserted at the surgical site. It connects to a softball sized sphere that is full of the pain medicine - not narcotic, but an anesthetic kind of like novocaine or lidocaine. The sphere slowly compresses on itself until it is empty. This can take several days, so the anesthetic is continual. You wear a little pocket with a strap to hold the sphere. The tubing is often sutured into the skin to prevent it from sliding out accidentally. It is used for surgical sites that are extensive and painful - maybe like a big hernia repair, an open gall bladder removal, or like my chest surgery where I also had a chest tube and intercostal nerves were involved. It is not used that much for basic laproscopic surgeries which have minimal pain compared to open surgeries. The sutures on mine were very bothersome. Every time I accidentally caught the tubing on something it felt like my skin was being ripped off. I've never heard of someone having it for laproscopic weight loss surgery. The pain associated with WLS is from gas (Gas is put into your abdominal cavity to expand the field of view. It is sucked out, but any residual gas has to dissipate into the tissues on its own and that can be uncomfortable. I had NO gas pain after gastric bypass.) The other pain associate with WLS is surgical pain of the procedure itself. There is a lot that goes on underneath those little laproscopic incisions. But it is not unbearable. For comparison, I would say my broken pelvis and ribs from a horseback accident were MUCH more painful. That was a 9/10. My thoracic surgery was a 6-7/10. My total knee replacement was a 5/10. My gastric bypass was 4/10. I needed morphine maybe twice in the hospital, percocet for a day at home, and then tylenol for about a week.

Congrats Ginger and thanks for sharing. I had my revision on 7/25. I've had no issues with GERD. My doctor has me on Prilosec, 40mg for the next couple of months. Stomach is still sore and swollen, but i get my staples out on Monday so that's a plus (i think [emoji3]). So far I've lost 20lbs, YEAH!! WELCOME TO THE RNY SIDE!! Your fasting time is up to you/your lifestyle but people usually find fasting late evening- overnight- into the late morning easiest because youre sleeping most of those hours and not focused on any activities/food BUT Beginning IF so early after your surgery (for anyone really) maybe tricky Here are a few questions that may help guide you to some answers? (Others please chime in/correct me if I'm wrong) 1. What were the reasons for your revision? *Poor habits that lead to regain: no, not yet. You may need fix this aspect first (You may not need to use IF for weightloss at all if this is resolved π) - go by your Team's plan first then discuss when it no longer works/weightloss slows/stops *GERD and you're at goal: Maybe if you are looking for inflammation reduction - discuss with team first and go slow *Other: discuss with Team first 2. Do have significant regain to lose? *Yes: Go by your Teams plan first then try IF when it doesn't work anymore (under their supervision of course) *No: you may lose more than you want so use mildly 3. Are you getting all of your nutrient/macros/cals in easily/too easily? *No: I wouldn't try yet- it may just make it harder. *Yes: If you over eating, go back to just dense proteins only for a bit to see if it helps, if not discuss with your Team the prospects if IF for hunger reduction/calorie reduction Hope that helps π -

I watched The Boys (Amazon) π Boring food photos this month because not much variety in my menu. But shameless selfies abound π

ππππππππ Updates ππππππππ 24 days post op & doing pretty well. Lots of vitamins in my routine. I also realized that i need to open my PPI and swallow the beads as opposed to taking the capsule whole otherwise I get strong reflux like symptoms late in the evening. I don't think it GERD returning but post op I was placed on the PPI for 3 additional months to calm things down the evaluate. Also figuring out my new food intolerances and so far just heavy cream based soups (will not try again) As far a diet i am progressing on schedule but I have had beans and tubers which is not on my NUT plan. She failed to take into consideration: 1. I'm already at goal (she wanted me to eat pure protein nothing else π) 2. I have prior protein intolerances so they need to be rotated in/out or I get constipation/aversion 3. She hasn't dealt with a revision patient that was already at goal, didn't have regain So I kindly injected the food in the appropriate stages that I remembered from VSG π (yup I still have my packet almost 2 yrs later) β°β°β°β°β°β°β°β°TMI TIME β°β°β°β°β°β°β°β° *squeamish please exit stage left* At one point last week I experienced terrible constipation from eating only/too much protein and called my team to confirm the plan. I was told to increase the stool softener and laxative until it resolved. If that didn't work I could use MOM or an enema. I had to use all of them to fix it π. But it worked and I have been regular ever since This week was much better, likely from the natural fiber balancing out the protein plus increased SS/L, however today...diarrhea. No good reason except my cycle is approaching. Oh Joy. This has reduced my weight to 150lbs. I just need to make certain this isnt a stomach flu or lasting thing as I have a Wedding next week But so far so good π -

OK I get the intermittent fasting method burns better than regular eating. My problem is Iβm confused on whether to do the fasting during the day or at night. So confused I donβt know what to do. I am 10 weeks out from a revision from sleeve to bypass should I be trying this fasting right now or is it only during maintenance? -
9 months ago guys I was at my heaviest weight ever at 304 lbs. Today 7.5 months post surgery I am currently at 179 pounds. I am so happy and so proud of my self! you should all be as well. This has been a great journey so far with no negative side affects so far and I wanted to share it with all you guys!! Hugs to you all!!! π

Follow-up care for surgeries done outside of the US

First, there is a specific Mexico/Self Pay forum here that might get more or better answers. I can't speak specifically for any of the MX practices, though my wife was self pay here in the states, Most follow up care is done by your PCP including routine labs, and those are generally covered by insurance. From what I have seen, most US bariatric practices aren't keen on doing follow up for other doctors (partly not wanting to support the competition, partly not wanting to get involved in someone else's surgery and potential problems) though longer term if revisions are needed, they are more than willing..... I would expect that the better practices would offer some kind of remote follow up (phone/fax/email/skype, etc.) for some time, but don't know for sure on that. The practice that we used here in the states was set up for travelling patients, so that is routine for them to do the periodic post-op follow ups as well as the annuals. One can also often join the support group for local surgeons to get that kind of support - many welcome patients from other practices - and that may also be a way in to see them if there are any problems that need attention (ask questions at the group meeting - I'm having this or that problem - and that may interest them enough to invite you to make an appointment, whereas they may refuse if you just call the office cold. -

Sleever's don't have the pouch like our gastric bypass friends do. Most of these online reset programs or "reboots" attempt to shrink up the pouch, and from what research I've done most of them don't work. Once you have stretched it.. its stretched. The good news is, its difficult to stretch. The fact that you can eat way more 2 years out is normal. Has little to do with you having stretched out your stomach and more to do with you just returning to your ability to eat a normal meal. Post op I was eating like 500 - 800 calories a day for the first 8 months. that's normal for bariatric patients, but its not normal AT ALL. Your body can maintain that for a short while, but at some point we have to get back to normal sized portions and at or about the 2,000 calories a day target. That said... a reboot isn't a bad thing. But I see it as a mental journey, not a physical one. Do things that will remind you of why you started down this path to begin with. Start with the pre-op liquid diet for 10 days. Remind yourself of the HELL you drug yourself through years ago to have the surgery to being with. Perhaps look at some old journals, or FB posts and goals you had set for yourself then. Have you achieved them? If not... why not? Its all about fighting for you. Gotta get back into that game mentally and start making better eating choices. Smaller portions of acceptable well rounded meals and you should get back on track. Long term success is nothing more than short term success repeated daily. One day at a time. From here on out, its a mental kick in the pants. Its the mental reboot your struggling to get a grasp on which is causing more carbs to sneak back in and perhaps some weight to come back. Know its all totally normal, and totally correctable. You know what you need to do. Now just do it.

Prior to that I cleaned my refrigerator thoroughly. It was completely orderly. Because I wasn't always cooking he usually ordered out and my refrigerator was wrecked/stuff to the gills with things no one would have eaten for leftovers. B-Side Questions: How often do you clean out your refrigerator/pantry? Anyone in particular that ruins it?

My revision from sleeve to gastric bypass was May 23, 2019. I am still having to battle my addiction to sugar. Itβs a tool and sometimes were able to cheat but I have to learn how to stop emotionally eating and kick the sugar out the door.

I got my first real bout of the foamies. Gastric Bypass July 1, 2019 lost 41 lbs. Really struggling tonight after eating toom much coconut soup with chicken. Think I was eating too fast AND not chewing it up well enough. UGH

The same thing has happened to me. I was thinking about getting the revision surgery. I'd rather not go under the knife again. I know that you can do the diet from the first month to shrink your stomach again. Im m not sure how effective it is though.

I'm hoping someone can help me out with some suggestions please. I had a gastric bypass 14 weeks ago which didn't go to plan and I had to have emergency surgery to repair my insides. I was left with a drain in for 6 week, which left me having to take antibiotics because it was leaking infection. Once the drain was removed I thought great I can start my new life. Unfortunately it hasn't turned out that way, as soon as I start to eat I feel sick, I go through the motions but nothing comes out this lasts between 5 minutes and 5 hours, I have even got anti sickness tablets which haven't helped I feel and look like an old woman even though I'm only 52 because of this. I hate looking in the mirror because a gray faced woman looks back, I feel weary and very ill because of it. It doesn't matter what I eat because I get the same reaction every time. I've been so poorly my mum has stayed with me for 12 weeks. I haven't had any follow up letters from my surgeon yet so I don't know who to talk to because my own doctor isn't clued up on it so doesn't know what to advise. Yah, I've been tested for anemia. Apparently OH is an effect of gastric bypass... it's not unheard of. The rapid weight loss throws off the autonomic nervous system. The surgeon's office thinks it will eventually level out once the weight loss totally stops, but it's not a "sure thing".
