  1. Dumping, when it occurs, is primarily a function of rapid stomach emptying owing to loss of the pyloric valve, rather than the intestinal bypass portion of the procedure. The DS typically doesn't dump because it uses a VSG as its basis (so it preserves the pyloric valve) but it has a very aggressive malabsorption component. I don't know what the common practices are these days on RNY limb lengths, but it is specified to some degree by the insurance billing codes which define the standards of care for the procedure. When my wife was getting her WLS years ago, the surgeon mentioned at one point that when he did an RNY (usually because the patient's insurance wouldn't cover a DS) he liked to make them as malabsorptive as possible within the limits of the CPT codes. There is what is called a distal RNY which is much more malabsorptive than the standard proximal RNY that most get, but it is rarely done (at least in the US) as insurance rarely covers it. Within the normal proximal RNY world, there are some tradeoffs at play (when isn't there?) between amount of malabsorption within the legal limits on the long bypass side, and the threat of bile reflux is things are made too short. So, while it's a good topic to discuss with your surgeon if you are interested, I would go with their experience on this to get things right for your case.

  3. I have no idea how long the intestinal part of my RNY was bypassed, and I bet most people don't. BUT...I just wanted to add that most RNY'ers don't dump (the number I see thrown around a lot is that 30% of us dump, but I don't know if there's any actual medical research to back that statistic up. But based on what I've seen here and on other internet forums the last few years, I wouldn't really doubt that statistic. Lots of us have never dumped. And for those who do, you can control it by limiting or avoiding sugar, which we should be doing ANYWAY. also, most of the people who have malabsorption issues aren't being diligent with their vitamins. It can happen to those who ARE on top of their vitamins, but that's pretty rare. The only exception to that is iron - some people don't absorb oral iron very well and end up getting occasional infusions. Although even that is unusual - the majority of us absorb oral iron just fine.

    I think the phrase "go away" is a little of a misnomer. Our bodies adjust to whatever circumstances it is presented with. In the case of bypass, the GI tract has been re-routed to intentionally result in malabsorption. After a while, the body adjusts a bit to it and will learn more efficient ways to absorb nutrients. That's not to say you don't have some malabsorption forever and it's not like it re-grows your intestinal tract or anything, it's just become a little more efficient at getting what it needs. The body is amazing at overcoming deficiencies like that. It's not quick at it, but it happens. You've heard of the "honeymoon" period of WLS? That's kind of the theory.

    I probably read over it, but why did you revise? I was a bandmember from 2009-2017 and lost about 89#. (232#-143#). Lost it due to a slip, due to a stomach flu. Was WLS free for 2.5 years and gained 30#. Day of sleeve 8/28/19 (tomorrow is 9 mths) 173#.... today 126#. I'm worried about regain with the sleeve. funny that i was never fearful of regain with the band. I had much better control with the band than with the sleeve. I've done great with the sleeve.... but i seen how people slowly increase their food and regain. Right now i'm still at the 2-3 bites and i'm full, but i'm hungry again sooner. With the band i was hardly ever hungry.
  6. So 12 days post op of my revision from band to sleeve I woke up to a 102.9 fever. Went to ER after talking with my Dr. once I got to ER after blood work I was Being admitted and treated for Sepsis. After more testing I had a leak. I spent 6 days in the hospital had a Stent place in my stomach and the abscess removed. I’m certainly at home on day 3 with a TPN bag. Absolutely nothing by mouth and IV to keep me alive pretty much. I have a very long road to recovery.
  7. Had my RNY gastric bypass 2 days ago. I’m home and other sore I’m doing pretty good. Hang in there everyone your time is coming and it will be worth the wait in the end. Best of luck to everyone!

    Well if you're not using calories then you need to adjust your intake to suit.. But otherwise talk to your Dr if you need a fill? Part of having surgery and a new lifestyle is figuring out how to adjust our diet as we go along; keep a log book of what you're eating and then look back on it to see what changes you need to make I've had the bypass and weight loss has been great but I know one day the malabsorption will go away and I'll have to figure it out..

  10. I was always curious myself about the length of the bypass and why it matters? My surgeon said he did 120cm I think the longer it is the more chance of vitamin deficiencies?
  11. Hi Gang. Can the Bypassers weigh in? I'm scheduled for a Bypass on June 29th and just had my last visit with my surgeon who told me he would be doing a 100cm bypass. Does this seem short? My concerns regarding the Bypass were the malabsorption and dumping. I know some people value these to help them stay on track but I do not so I thought the sleeve would be best. But I suffer from reflux and have since I was a kid so Bypass was recommended. I believe my concerns about Bypass are why my surgeon chose this "100 cm size". However, from my research most people do twice as much. Thoughts on the pros and cons? Would love to hear from other 100 cm Bypassers. Thanks!

    Any re-sleevers out there?

    I found a study about it. Apparently some people have had success with it. It's called laparoscopic re-sleeve gastrectomy. I didn't quickly find anyone doing it.     Its not a stupid question. Honestly I do not know. I was told I was given the bypass and they didn't bypass as much as maybe others because I didn't want problems with vitamin deficiency. I was drugged up but I think I remember him saying the rest of the sleeve is still in there. I am going in on Thursday to find out just what is going on. I did see the swallow test the day after surgery and it looked like a straight line pretty much and a small bump in the middle, which I thought was the stoma but as I said I was drugged up. I will let you know as soon as I know though. Another thing though is I haven't eaten solids, so it is possible that liquids and mush just slide right on through. I am pretty sure Wendy will advance me on Thursday to a soft diet. So maybe I will feel something with eggs, cottage cheese, beans, ect . I am hopeful. Either way I am back to tracking food like I did with the sleeve the first 18 months and weighing and measuring.

    June 2020 surgeries

    Got my date! June 29th! Doing Bypass after a lot of back and forth. Still room to switch to Sleeve if I change my mind but just spent an hour with my surgeon and he really sold me on the bypass. My BMI is 36 and my EGD looked good so he said it was my choice but he considered Bypass to be the Gold Standard. I worry about dumping and malabsorbtion but I struggle with GERD so... I hope it's the right decision. Now to find a protein drink I can tolerate. Glad to join this group. I'm keep this a secret from everyone except my husband so I look forward to finding the support I need here and to be some support in return. Onward! ~ Meow.

    Revision time

    Same here. I also remember one of the dietitians claiming it's not that problematic with bypass but more with the sleeve as liquids pass rather quickly through the anastomosis. Can't remember the exact words because it's 4 years ago now. I know the 30 min rule but noticed early in that before eating the drinking up until a few minutes before the meal is no problem. After the meal drinking too early is just making me feel uncomfortable. Sometimes I wait longer than 30 min. Sometimes it's just 25 min or something like that. Depends on meal size and meal consistency. Mind you, this is not how I handled it a few days out. Take it slow.

    That is what I am going to do too and I know I can do it . I am just not happy about it and I don't understand why some get restriction with revision and not others and no one told me from the Drs. office. They had me down at hospital to be taking in 8 oz of full liquids each meal. Really ? I could not understand why. I cannot wait to get to my appointment Thursday. I weighed 296lbs the day of my surgery ( sleeve) and in 2 months I only lost 27 lbs. I hung in there and made goal by following the plan the first year. my original goal of 180. I went on to loose down to 164.8 the lowest I ever got and stayed there all last summer was on my way back down and decided to hold steady thinking there was going to be some big weight loss and I didn't want to be too thin for my frame, SHEESH had I only of known, lol. Went in Friday weighing 180 and came out Sunday 185. This morning 181 lmao

    I had my revision in March and lost 40 pounds. I had my sleeve in March ( 5 years ago ) and on this date lost 55 pounds so it’s definitely slower and I’m actually exercising and counting calories etc this time around. I’m trying to restrict myself just like the sleeve so I’m only eating protein and a very small amount at each sitting and drinking a lot a lot of water. It’s hard because that never full feeling is depressing. It makes food and eating ALL I can think about. I’m trying to occupy my time with other things also

    Umbilical? right ... I had a hiatal fixed/removed during the sleeve, or maybe It was removed w/ the stomach LOL I don't remember, lol I am unsure what time - waiting for the 'call' today - I had to get a rush COVID test yesterday - I was anxious about that as well but it's negative. I had a horrendous dream, id get to today and find out otherwise!! EEEK!!! My husband had the virus back in early March - was exposed, no antibodies though - my daughter, 17, had a high antibody rate and NOT ONE symptom, so weird I tell you! I'm hoping it's early morning - unsure how many surgeries they are doing tomorrow.. I know he had 2 available dates upcoming since they removed the ban on electives... I'm one of the first surgeries to go now with the hospital opening up - they made sure I was okay knowing there is a COVID floor - but I believe that they are going to be so cautious now and soooo extra clean with everything.. I just hope I don't have to wear a mask in my room, that'll stink!!! They are horrible to breath, esp with the fogginess of the reading glasses. I'm sure bypass will be fine - have had surgeries before, and two c-sections and managed lol The hernia is scaring me, as I Have to be extra careful they said about lifting this time...

    I will tell you I had a hernia repair also with this bypass and I was in more pain then with the sleeve. They managed it with meds the worst I was was an 8 and that was after I started coming to so its foggy. I know exactly what you mean I also just wanted it to be over with and be home and I am doing much better home then in the hospital . I am surprised I am not tired and hurting more to be quite honest. I have been home a little over 48 hrs and only taken 3 pain pills. You are going to do great and we can start getting this little extra weight off together ! What time is surgery ?

    yes, we will be pals I was sleeved almost 5 years ago --- did well, gained back a little, but this isn't for weight loss - the GERD hit me last year and I'm on double PPI now and it's horrendous - I have a umbilical hernia, not related, maybe they claim, but thats being repaired too - so maybe thats why I'm worried. I did great with the sleeve the first time around - pain, etc... I'm just worried this is a double whammy, the bypass and hernia fix. I am not looking forward to it - nervous and anxious ... I know it'll be fine -- just the high of getting it all done .. I just want it over with if you know what I mean. I was so prepped in March, excited, now with all this COVID and being put on hold - I just let my guard down -- worry no one can visit ... it's just a lot going on in my head... anyways, I'll be okay!! I'd love to lose another 40-50 lbs, but if this GERD goes away -- AWESOME!!! that's what I'm waiting for. Water gives me heartburn... its constant all day .. I'm glad they cleared the way here in NY for electives. I was on the emergency list, so I've waited only 2 weeks.. Today is clear liquid - horrendous!! LOL

    Good luck to you and I truly mean that. I will be watching for your progress and maybe we can share being so close together in revision surgeries. Ya know I had read this thread before having the revision and heard the girl saying she had no restriction and I either commented or thought everyone is different and that maybe you don't feel it yet because we are numb from just being cut on ? I hope that is the case. I would like to lose 20 lbs so hoping this malabsorption thing is for real with this bypass. On a happy note if you are doing this because of GERD I haven't felt a thing ! I cant say its gone yet as they have me on my PPI for 3 months. I do believe it is gone though I have had no burning whatsoever !

    I will be there seven days total arrive the 14th surgery the 15th depart the 21st it would be great if they thought I was ready to have the drains removed before I left. I will have to wait and see. I had some issue with the drain post gastric bypass but they did remove it before I left hospital. weird I know but what I mean by not relocating my belly button is just not having one. Because honestly when they relocate it, it’s basically creating a faux belly button. At least that’s my understanding of it but it’s another wait until I see the surgeon and go from there sort of thing. But for me personally I couldn’t care less about belly buttons and one less thing to worry about keeping clean 🤣

    Obsessing about Plastic Surgery!!

    It’s been almost 13 months I guess since my surgery do I have patience for waiting for skin surgery not really. I think my weight is stable enough now. I think having the sagging wrinkled skin is uglier than any scar could be. And I too have had quite a few abdominal surgeries. Hysterectomy in 2010 and then Two pelvic laparoscopies that we’re sandwiched around my gastric bypass surgery. I’ve had other surgeries in the past. I lost count on the foot and tendon surgeries. I have a long incision at the back of each calf. And breast reduction which feels like a million years ago. It’s not the stretch marks as much the shar pei hanging wrinkled skin and the flab as you put it that doesn’t go away but hangs. I took the dreaded look in the mirror at my rear and I’m hoping that I can have a body I haven’t had as long as I can remember I’m fine with some scars ... I don’t need it for attention but for how I feel physically and mentally. But it’s hard to put to words. my one concern is making sure the incision site doesn’t get Stretched. Has anyone found silicone tape helps with keeping tugging on incisions? Actually my biggest concern is my allergies to adhesive, iodine, and surgical glue.
  25. i'm being revised tomorrow and I'm a nervous wreck!!! Unsure why - been anxious for a few days.. I was so ready in March, then April, and here I am and nervous!
