Hi, my name is Krislynn, Kris, or just K., I'm new to this forum. I found it researching more about the duodenal switch. I turned 42 in July this year, and my highest weight before my first surgery was 536. I had the gastric sleeve in June 2017. The first year was a great learning experience in learning about healthy foods, changing my mindset, and what I needed to do to get healthy. That first year, I lost 221 lbs. But, then when I went back for the 2-year check-up, the surgeon that took care of me, had left the hospital and program and they informed me they had no one for me to see. But, then Covid hit shortly after that-- then I lost my job and ended up sitting home 2 years and gained close to 200 lbs back of what I lost and that was crippling to me because all my old health problems came right back. during all this happening, my mom and I chose to move from California to Indiana to be closer to my sister, I decided I couldn't take care of my mom who has declining health because of her weight too, has been type 2 diabetic for more than 20+ years at this rate with my health starting to decline again too. So I reached out to the weight loss doctors here, I spent 6 months Feb-Aug '24 taking off 70+ lbs by following their 1200 calorie diet, getting back into a healthy mindset. It's a struggle at times, and sometimes I take two big steps forward, and then I end up taking a big step backward, it's a daily walk. once I completed the mandatory 6-month medical weigh-ins, things moved quickly, I got approved by the psychologist, and then the insurance approved me to have the Duodenal Switch or SADIS surgery quickly, within a week I heard back. My new surgeon who's great, got me scheduled quickly after I finished all the required testing, and preop classes, this Monday, the 11th at 7 AM. I am feeling nervous because even though every test they've made me do has come back normal and negative, I am 7 years older and worry that something might possibly happen. I keep reminding myself, that I've already had the sleeve, so I won't be under that long, just long enough for him to do the switch. so basically, I'm going to go to sleep and then wake up on the other side of it. Can anyone who has had this surgery, give some advice or maybe just let me what to expect after this kind of surgery? The last time I woke up the only thing I remember is the immense pressure I felt on my chest from them feeling my stomach with gas to do the surgery. And The surgeon told me unfortunately I would be dealing with that again because it's something unavoidable. So, I'm prepared for that. Love to everyone, Krislynn☺️💜

Mini Bypass reversal

I think you mean revision. A reversal would mean they rejoin your digestive system to what it was before surgery i.e. back to normal anatomy. (It’s high risk and your digestive system may not function the same or as effectively.) You can’t reverse a sleeve. But if you’re going from one type of weight loss surgery to another where they alter your digestive system further like sleeve to a bypass, or bypass to Sadi, etc, that’s a revision. It may change the responses you get unless you do mean reversal of course. 😊 Revisions aren’t uncommon, for various reasons, but haven’t heard of a reversal here. -
it took me 4 months (and ~15 lbs) to figure out my maintenance calories were. i also had to get over my fear of gaining all my weight back and actually start eating MORE. maintenance level calories are VERY individual, and vary greatly from person to person. and even then, it varies greatly form time to time for the SAME person. 4-ish months after achieving goal, i finally figured out that i needed 1800 cals a day to stop losing weight... AT THAT TIME. over the years, this amount has changed depending on my activity level. luckily, i am anal and i track my food intake and weight DAILY so i am armed with data to figure out what i need at any point in time. i am 6 years post op now, and these days i need about 2300-2500 to maintain my current weight (i exercise alot). had i continued to stay at 1800 cals from the before days i would have shrunk beyond what i wanted. soooooo....long story short: your maintenance calories will change depending on your output and input, on your sleep patterns, overall health, stress levels, age, genetics, etc....its up to you to notice what affects you and act upon it. so, so, so, so, many times i read folks on here saying "i gained/lost weight and i just don't know why!". the answer is PAY ATTENTION....you will know why. P.S. i do know that "normal" people don't need this level of attention to stay at a healthy weight. i also know that i am NOT a "normal" person. i mean, i got to a BMI of 43 (from BMI 23), in 10 years. it takes more "effort" for me to stay at a healthy weight that a "regular" person. so i do what i am willing to do.

as others have touched on above, it depends on what you call a "normal sized meal". can i eat my "normal sized" pre-wls meal from my obesity heyday? hecks no. but, let's be real, those meals were bigger (and not as often-consumed) as the meal sizes of not-obese people. now...can i eat enough food to maintain my current weight, feel satisfied AND not feel like i'm depriving myself? yecks, YEAH. from the outside and to those not in the know, it probably looks like i don't eat enough, but its enough for ME. my restriction is still very much in effect, so i don't eat alot in VOLUME at one sitting. but i did* manage to get my 2300+ calories in by eating often, and choosing higher calorie foods. (NOTE: i exercise ALOT, so my caloric needs are probably higher for your average 5'2" person, but still) *clarification (and to go off on tangent): i WAS able to do this, but lately am not eating as often - for other reasons** - so my average calorie count has been dropping, but i'm working on it.... ** i got braces and my pure HATRED and annoyance of having to clean my teeth after eating ANYTHING has effectively stopped me from eating. it's in my head, i know. i'm working on it.... -

Oh yay! I used to use it but good to know if or when I go back to it! When is it ok to go back to your movements? I finally made it to the gym today and did a body compat class .. I'll attach my chart! It was a full body workout like a mix if martial arts.. excited for my weight in -
I wish I could say from experience but I never made it to the maintenance phase with my sleeve and I’m early out from my revision. From being on the boards though it seems what most people do is to add like 100 healthy calories at a time and wait and see if that makes them gain and keep upping it until they find what their body is happy with. Of course if you level of activity or type of calories changes you may have to adjust so I think if you don’t choose to log your food daily you may want to at least spot check it like once a week just so you can see if your habits change gradually and keep on track for what works for your body. Congratulations on nearing maintenance!!

Everyone is different. I am almost 6 years out. I made it to my goal weight about 1 1/2 years after surgery using the Fast Metabolism Diet for the last 6 months of that. I went back up 15 pounds but my body is happy here. I have never thought about calories. I have always concentrated on how many ounces I eat at a meal. I never go above 3.5oz total. 2oz protein & 1.5oz. I also eat 5 times a day. 3 meals & 2 snacks like cheese or fruit. You have to figure out what works best for you.

Nope there’s no normal. You have to work out what you need in regards to nutrients and calories to maintain & your body to function effectively. Gender, height, age, weight, activity levels, how your body functions, etc. all play are part. For example 2000-2400 calories is the recommended calorie intake for a woman. If I ate that I’d be as big or bigger than I was before surgery. In fact I never ate that much when I was obese. When my weight first stabilised I ate about 1300 calories yet a year or two later I began consuming about 1600 & still maintained - my needs obviously changed or stabilised. If I weighed more, was taller, younger, more active I would need to eat more. As to working out what I needed, I just slowly ate more as I was able until I stopped losing and my weight stabilised. A basal metabolic rate calculator can give you an idea of what you may need calorie wise but much like BMI calculators they’re not perfect - don’t consider genetics, health needs, muscle mass, etc. I found the one below pretty on point for me but … https://www.calculator.net/bmr-calculator.html

Exactly as others have reacted. What is your definition of a normal size? I can eat about a recommended portion size and that took me well into my 3rd year. Sometimes it depends on what I’m eating or if I’m feeling hungry & then I can’t eat the whole portion. Not unusual to have leftovers in my fridge. As an example I can eat about 3ozs of protein plus a cup of vegetables for dinner, 2 golf ball size rissoles/meat balls & cup of vegetables, about 2/3 of 2 scrambled eggs, a tub of yoghurt, 1/2 a large apple, 8 macadamia nuts, 8-10 grapes. If I go out to eat I can eat about 3/4 of a main meal in a fine dining restaurant when meals are pretty much recommended portion sizes. In Australia our servings aren’t as large as some of the American servings though they are getting larger in the casual eating places. For example, I ordered a small salt & pepper calamari meal at a pub & took half home for dinner. This is an Australian site (below) but I’m sure there’ll be US sites about recommended portion sizes to give you an idea. @ShoppGirl how did all your medical stuff go? Are you doing okay? @Justarwaxx I have heard periods get worse when your losing weight because so much estrogen is stored in fat, so when you lose fat you have more estrogen in your body, which in turn makes you cramp more and your flow worse. That has been my experience so far. I was hoping I would have lighter periods since I'm not intaking as many calories. -
Hey, i do have a question about Maintenance calories in general, I lost around 135kg's in weight in the first year, and im "slowly getting there" and even considered recreational surgery now, but i have problems with the idea of Maintenance Calories and goals. So thats why i ask here now. Is the General Maintenance kcal after the surgery lower than the ones for a normal "person" ? or do we kinda "slip into" that?. i know all the math about calculating it, but is the normal Calorie intake calc. for people with a rny Bypass even a thing?

My question to you is why would you want too? The whole point of having WLS is to make us healthier by losing the weight AND keeping it off! What's to keep people from going down that slippery slope once again if we could eat as much as we once did? Enjoy the portion restrictions as a way to maintain a healthier lifestyle! -

I'm 21 years post-op. A sandwich that I could easily eat pre-op is 4 meals now. The thing is, my concept of 'standard' has changed completely. For this thing to work long term it had to*. In the years pre-op I could do any crash diet that came along with 100% compliance for months. I can do anything for a short time knowing the short time would end eventually. That's not change, that's will power, which most of us have in abundance. WLS is forever*. The early "honeymoon" period shouldn't be about losing weight as fast as you can*. It should be about changing "normal" and getting weight loss as a positive reinforcement for compliance*. Compliance + positive reinforcement + time = new normal*. My plan was to eat the way I would after I lost all the weight. Basically, I started my "maintenance" plan the day after surgery. I eat now pretty much as I have since about 3 months post-op, though now I complete more meals than I did at 3 months. There is no "after." This is my normal, the way I eat without thinking. WLS is forever is my opinion. Plenty of folks have done the draconian post-op diet and lost all their weight and even maintained it for years. Many track every morsel they eat forever. For me, that's no way to live. This is not how my surgeon wanted me to live. Good luck, Tek * Personal opinion. -
Hi everyone! Is anyone still around in this group? Its been 4 years since my surgery, and what a ride it has been. I have had ups and downs, and now I am trying to work on myself a little mor and loose some more. I had my tummytuck done in April and in three weeks I will have my inner tight surgery. To keep the weight down I have used Ozempic and Wegovy. Would be fun to hear if anyone is still around and how have your been since surgery!

Gotta love kids. They are so brutally honest. But of course compared to a 14 year old they are flabby. I am suddenly very aware of my saggy arms too and I have only lost 59 pounds so far. I think part of it was just normal aging and I just wasn’t paying as much attention before to them as I suddenly am now. My belly skin is starting to get pretty jiggly too. I am dropping the weight rather fast though. BMI of 39.2 to 30.3 in 3.5 months. My secret goal is another 51 pounds so I’m sure it’s gonna get a lot worse if I can get there. Unless insurance helps, plastics are not an option. I’m really hoping it snaps back a bit with time but I will be doing a lot of crunches and pushups I’m sure. Does it make it easier or harder to stand back further? I am just doing them exactly how the instructor showed me in class which is my feet back a ways but not as far as you describe. I haven’t told her yet they are getting a bit easier for me now. I assumed the next step was knee pushups, but they hurt my knees so if I can alter the wall ones and skip the knees that would be ideal.

A good tip would be to have a mix fat and carbs every couple of hours to help you steady your weight so things like dates + tahini or nuts, Avocado milkshakes, full fat chocolate milk and a couple of tbsps. of peanut butter, Full fat yogurt with nuts, full fat milk with honey, just generally nothing low carb/low fat. For me personally, protein fills me up way too quick so maybe try high fat, high carb moderate or lower protein until you stabilize. Sorry you are going through this, please make sure to see your healthcare provider ASAP.

I rarely bought brand new clothes while losing post-op. I love thrifting and had a blast as I shopped for smaller and smaller sizes. I sometimes hardly had a chance to wear some of the clothes I bought before I outgrew them. I go through my closet once a month pulling out what I have outgrown and donating. It's slowing down now as I near goal weight. When I was at my largest I could never find anything to fit me locally. I mostly ordered online and even then a lot didn't fit right. 4-5x Funny now that I'm in a smaller size, I'm finding cute plus size stuff. Go figure. But I ain't complaining. Here's a pic of my latest closet clean out. Yeah I wear a lot of black. I'm trying to change that but boy a few of these items were hard to part with. I did find a loved one who wanted a bunch of it so that made it a bit easier to let go..... -

I had to do the liver shrinking diet pre-op for 2 weeks. 2 shakes a day then I ate a bag of frozen stir fry vegetables with 3 ounces of lean chicken or beef season fried in soy sauce for my evening meal. It was delish. I lost weight. High volume, low calorie. I'd enjoy the food while you can. JS -

No! No! No! Besides why would you? Let’s be honest. We enjoy eating. The smell, the textures, the flavours, the look and sometimes the sound of food ( like the crunch of an apple) is appealing & gives us pleasure. We celebrate with food. We share food with loved ones. We give thanks with food. Why would you sacrifice that to drink a protein shake for every meal? Yes, you may be prescribed shakes in your pre surgical diet and you will be prescribed them for usually 2 weeks after surgery. The pre surgery shakes are to increase your weight loss to improve your surgical success odds & to shrink your liver so the surgeon can see the surgical field more easily. Post surgery it’s to support your healing & not strain your digestive system (remember all those sutures & stables holding it together). Short term reliance on shakes. The goals of the surgery include you changing your relationship with food and establishing a healthy, nutritionally dense, sustainable way of eating. Drinking shakes for any extended period of time is simply not sustainable. It’s not nutritionally sound, only gives more power to cravings, creates new bad relationships with food and will highly likely result in weight regain & poor health. Key word here is supplement. If you are struggling to get your protein in via the real food you eat, sure a protein shake can be beneficial as a supplement to your earring. Same with any vitamin or supplement. If your body is lacking in specific vitamin or minerals, sure take a supplement but they should never be what you totally rely on to get the nutrients your body needs to function unless you have a specific medical condition that means you can not eat real, solid food. I never touched another shake after I began purées (start of week 3). My goal was always to get all the nutrients I needed through real food. And I do. I don’t even take vitamins now (except in winter when I have a dip in my vitamin D - I feel the cold so hibernate in winter & rug up if I have to go out so little sunlight for me then). Ask your nutritionalist, surgeon & GP. I bet they agree with all of us. -

I am asking for your help/advice. I am a physical therapist and I underwent my own bariatric surgery in 2022. I am looking to develop a unique bariatric program that provides exercise programs and support to people undergoing rapid weight loss whether from surgery or from medication. If you guys could answer a few questions for me it would be greatly appreciated and would help me build my practice! 1. Did you exercise before surgery? And what was your biggest challenge? 2. Would you have benefited from a preop physical therapy appointment to develop a customize strength training program that took into consideration your current limitations and orthopedic issues. 3. how were you educated on post operative activity and physical restrictions? Would you have liked a week by week guide on what activities you can return to and when? 4. Were you educated on exercises to maintain muscle mass through your first year postoperatively? 5. What kind of exercises did you do postoperatively in the first year? Did you hire a personal trainer? Join the gym? Search online? 6. Did you develop any new or worsening musculoskeletal pain as you were more mobile and your body composition changed? 7. Let me know if you have any other ideas on how having a physical therapist on your bariatric team would have benefited you both before and after surgery. Thanks so much everyone. I would start the habit now of following your surgical team’s instructions to a T. I think most who have had long term success here did just that. I know it helped me immensely in my first year, when I lost the most weight, to do what they said rather than trust my own instincts which led me to being overweight in the first place. -

Taking care of your mental health doesn't have to cost money. You can talk openly with trusted friends or family about your body image issues and low self-esteem. Or you can post here in this forum where you will find hundreds of people who share a lived experience of navigating the world in larger bodies. You can heck self-help books out of the library. Or buy a notebook from the dollar store and start journaling about your thoughts. You can meditate with the help of free YouTube videos. Or listen to relevant podcasts. You can find free sources of help if it's important to you and you want to make a change. Then again, I don't know you, so I could be completely misreading the situation. You can ignore this advice if you think I'm wrong, or if you just don't think anything I've suggested is worth trying. But after reading your original post, I thought it was important to say that if you can't immediately reverse your weight gain, it doesn't mean your life can't be really good. -
Liver shrinking diet pre -op?

I have heard a lot of people talking about completing a liver shrinking diet some weeks before surgery. My surgeon has told me that because my BMI is only 36 I don't need to do this and that it is only for larger people. What's your thoughts on this guys? I carry all of my weight around my midriff, I haver super skinny arms and legs so I'm more inclined to think that my liver does need shrinking before my op. I am intending to do a 2 week diet consisting of meal replacement shakes and at least 2 litres of water. Those that completed a liver shrinking diet, what did you consume? Those that didn't were there any problems with the surgery? Thank you!
