Hi everyone I had my bypass on 1/28. So one week into the liquid postoperative diet. I'm feeling pretty good trying to get my walks in with 18" of snow. I've lost weight before and after surgery. My biggest issue is wanting to taste real food but it is only a week away to pureed food. Chocolate is good but every meal over does it. I get hungry but I think it is mental, old habits from boredom and stress. I'm just glad to have a group to identify with. -
Sadly, it happens - more frequently a few years ago (6-10) when most bariatric surgeons were still learning how to do sleeves than more recently, but I guess that there are always some who are still learning! The sleeve tends to want to bend in the middle, or form an hourglass shape, if it isn't done quite right, it may not yield a total blockage type of stricture, but it can leave the narrowing that can impede the flow and/or exacerbate reflux problems. While most surgeons in the US are now far enough up the learning curve to usually avoid this problem, knowing how to fix it can be beyond their experience, hence many prefer to go with a bypass instead. It may be possible to correct your sleeve, but you may need to find a surgeon who is very well experienced with the care and feeding of the sleeve construction. My suggestion, if you want to go for a second opinion (which I think anyone should do when considering a revision,) is to book a virtual consult with Dr. Ara Keshishian, who happens to be on the wrong coast for you, out in Pasadena, CA, but he has been doing virtual initial consults for years before Covid as he has patients all over the country. This will at least give you a reading as to whether this is a viable option in your case, or give you confidence that the RNY approach is the best. If a resleeve is an option, then you can decide whether to travel across the country, or seek out another surgeon closer to you who can do it. I would suggest looking for one who routinely does the duodenal switch (DS) procedure, as they tend to have the longest and most extensive experience with sleeves. I believe that there are at least a couple in FL, and several further north along the East Coast. If you do choose to proceed with the RNY route, do discuss things carefully with your surgeon, as there are tradeoffs in how he proceeds. Limb lengths, as suggested above, are a compromise as if they are too short to minimize malabsorption, you can be more prone to bile reflux. There are several Facebook groups that cater to total and partial gastrectomy patients (primarily for cancer or gastroparesis) and bile reflux is one of their common complaints. When I was considering such a thing a few years ago, the surgeon I was dealing with said that as long as he kept the limb over a certain dimension (80cm, IIRC) then they saw no problems with it. Hopefully, the surgeon that you are dealing with has enough experience on both the WLS and non-WLS side of it to know those tradeoffs. Bariatric programs that are associated with major cancer center hospitals readily "swing both ways" on that, but one that only specializes in bariatrics may not. I wouldn't worry too much about the malnutrition issue, as the RNY is very well understood; it is somewhat fussier than your sleeve in supplement needs but things are pretty straightforward on it if you keep up with labs and change things up as those dictate; it can be problematic for those who get overly casual about such things and let it slide - then you can get into trouble. If you fall into that camp, then I would try to do everything to preserve your sleeve and its greater flexibility; otherwise, the RNY is a good alternative. My personal preference, as I was faced with some similar decisions, is/was to stick with the sleeve if it is viable, as the RNY (or something different) is always an option for the future, but once one has an RNY, changing things gets more difficult, so options are fewer. Also on the option front, with the bypass, there remains a "blind" remnant stomach along with the duodenum and upper intestine which are unavailable for endoscopic evaluation or treatment (things much be done surgically.) As there are an increasing number of procedures that can be done endoscopically these days, and into the future, and I have already had one lifesaving endoscopy this is an option that I am keen to preserve, if at all possible. Short term, you may lose too much as you go through the high level of restriction that comes in those first few months after surgery. In that case, there are ways to "eat around" your pouch by basically doing all of the "wrong" things for your WLS - drinking calories, eating slider foods, higher calorie options particularly fats as tolerated. The tricky thing is to avoid making too much of a habit of it as the restriction does diminish over time and you can naturally eat more of conventional foods to maintain your nutrition

Today is one of those days where I am so happy with the outcome of not one, but two bariatric surgeries. I had a gastric sleeve in 2014 and most recently a gastric bypass in October 2020. I found the 2013 picture in my Google photos and figured it was time for a comparison photo! This is the first time I've worn jeans, with a shirt tucked in and a belt, since 2015. I bought the belt in 2018 and yesterday my husband had to cut off 7 inches!!

malnutrition with bypass is pretty uncommon as long as you keep on top of your supplements.

I was converted to RNY from VSG for structural (and other) reasons too. My surgeon just said we were doing bypass so I’m not sure if there any specific differences between that and what you will have done. It’s completely possible to maintain (and even gain) weight after your revision. The malnutrition aspect comes with how long of a bypassed Roux (or alimentary) limb will be. Your surgeon may choose to make that length shorter to prevent excess weight loss, but that’s a plan you will come up with together. That depends on what surgery you had, which I don't see you have listed. With my BPD-DS I am supposed to, but I haven't been all that good about doing so and haven't had any ill effects. I believe its suggested that bypass patients take vitamins regularly. I didn't think it was as important for the gastric sleeve since that is not a malabsorption procedure. Did you doctor ever advise you about vitamin supplements? I had my surgery in Aug 2018 and lost over 100% of my excess weight, but I regained about 30 pounds from a (too low) 133 pounds in 2020. Even with that was still below 25 BMI. My fault, too many sweets and high carb processed junk food. Still have good restriction, and I've lost 10 of that 30 pounds so far in less than a month eating mostly salads with custom no-oil vinaigrettes, and lean meat and veg dinners. My preferred proteins have been tilapia and premier protein shakes. If I get hungry between meals I'll have an apple, which fills me pretty well.

I had a laparoscopic sleeve gastrectomy in August of 2018 and have had amazing results, from 346 lbs to 172 lbs. Since November of last year I have been vomiting in my sleep sometimes and waking myself up. Also, I have always gagged after eating no matter the amount that I ate, even two ounces. I recently relocated to Florida and saw a surgeon here to establish care and he reviewed a recent CT that I had for a hospital visit for flank pain and discovered that my sleeve healed in a hourglass shape at the suture line and now my esophagus is narrowing from working so hard to push down food. I have an EGD and upper GI scheduled for next week and then next month he’s doing a laparoscopic gastrojejunostomy and partial gastrectomy. He said it’s basically the bypass surgery. Has anyone else had this happen? I’m so scared because I don’t have much left to lose and don’t want to end up malnourished but I also don’t have any other options, I’ll malnourish and end up with esophageal cancer and or obstruction if I don’t have the surgery. Any feedback would be greatly appreciated.

I'm seeing my surgeon next week and expecting him to recommend revision to bypass for my weight gain and Gerd. I'm concerned about the hunger some of you are describing, but I'm really concerned about not feeling the restriction. Does that feeling go away after the bypass revision? I had gastric bypass (RNY) in 2004. Over the last few years I’ve been gaining weight. Last year (2020) I dealt with sciatica and was unable to workout and, of course, gained an additional 40 lbs. I want to know how many people have had the band over bypass surgery? Successes, difficulties, etc. I’m scheduled to have the surgery next week.

That is what I am worried about. My husband work offers insurance however open enrollment isn't until November then it would be another 6+ months after that. Looks like I will be paying out of pocket. -
Are you a man? I am as well, and I lost about 105 pounds after bypass surgery. I had lots of loose skin around my belly that hung over the waistband of my pants. It looked like belly fat, but it was really just loose skin. If it's any help, I'm just about two years out and it has gotten much better, although it's not gone altogether. I think it takes our skin a bit of time to catch up to the weight loss. I've been told that weight/strength training would help, but I haven't yet tried that.

PCP’s negative response to my wls

Nope. Covid makes testing really hard. I've done physical therapy for dizzyness, did a bunch of tests, including an MRI of my brain and they came back normal, took meds for dizzyness and they didn't work... right now my PCP thinks it's either 1. Postural Orthostatic Tachycardia Syndrome (P.O.T.S for short) which would require testing by the cardiologist and they've been sent the order no less than 5 TIMES and have yet to call back either me or my doctor or 2. a spinal fluid leak (which would have happened from the lumbar puncture I had in 2017 after brain surgery when I got meningitis and it took the 45 minutes to get the needle in due to a back injury that is a result of muscle, tissue, and nerve damage. I have a scar on my back from that lumbar puncture. Spinal fluid leaks mimic P.O.T.S and can go unrecognized for 10 years before symptoms get bad enough to where you really take notice that something is wrong. MY PCP is leaning more toward that and I'm currently doing physical therapy so that my insurance will approve an MRI). I'm lucky in that my PCP actually had WLS. She had VSG several years ago and her son had gastric bypass so she's a fountain of information lol. She encourages anyone who wants WLS, but doesn't press the patients or even MENTION your weight unless YOU bring it up! It was one of the things I loved about her most when I first started seeing her and I respect her so much for that. -

@rusnj1 I had my bypass on 12/24/20, currently down 23 lbs since surgery. Yes I was feeling the same way, but when I spoke with both my nutritionist and my dietitian they stated that was good weight loss. I mostly definitely get were your coming from especially when some people that I have seen lose 15 lbs a week, it can be discouraging but try to stay positive. -

Yes, many different ones including $3000 hospital grade down to the cheap $35 one insuranceprovides for at home use, ithe all only gave a few hrs of relief thats why i had 7 surgeries and they all failed tgen gastric bypass which getting weight off it is the only thing to decrease my pain at all and increase my mobility and stamina. -
I had my Gastric Bypass surgery in August 2019, surgery was a breeze but have suffered with sickness and nausea for the last year but due to Covid haven’t been able to see my surgeon. Was prescribed Odensetron which helps somewhat. But regardless of the sickness and not being able to keep foods down I do not regret having it done at all. Have lost over 140lbs . I am going to be celebrating my one year postop gastric bypass surgery at Disney World! Problem is I’m trying to figure out how to stay hydrated and eat healthy snacks every two hours so that I don’t lose weight on this trip because I’m already under white. Any suggestions?

Hi, I have been dealing with hypothyroidism most of my life. I had the bypass done in December 2020. I was 98kilos and now 69. I also have been back to hospital due to gore bladder removal and malnutrition. I am losing weight to fast to quick. I feel sick all the time and find it hard to eat food. I have not started doing exercise, I know I should. I go back to surgeon tomorrow so have a lot of questions. You are doing well, are you on to full solids yet? -
Hi, I am new to the group. I had the Bypass done in December and have been sick since. I just got out of hospital after my second visit due to the gorebladder removal and malnutrition . I was 98k and I am 69 now, lost to much. I have no desire in food at all, my stomach feels sick. I go back to the surgeon tomorrow. Has anyone else had any issues? Thanks

3rd Wk post-op overdone it walking too fast

HI, I don't know what I was thinking. I felt good? Anyway, a couple of days ago I walked over a mile 3 days in a row and walked as fast and hard as I could. That was a huge mistake. I wasn't thinking about my stomach bouncing around and not only did my Dr, do my gastric bypass he did 5hrs worth of work on my messed up intestines. I didn't do anything the past 2 days I was hurting so bad, not to mention yesterday I ran a fever all day that didn't get above 100.4. I was going to call my Dr today, but the fever is gone, so that is a good sign. However, not only am I so sore, I have lost my apatite, not my taste. Also, I hadn't had any problem with heartburn, but today it has been so terrible I took 2 of the Omeprozole pills. That didn't even work. Any insight to what might be going on with me? I would so greatly appreciate any feedback. Thanks Rhonda -

well, I just had my gastric bypass on Jan 12th and I wish I had not overdone it. I went to a basically empty mall where I live and people go there to get there walking done inside. I am only 3wks post-op, and not mention the Dr did 5 hrs worth of work on my intestines, besides the bypass. I walked 3 days in a row as fast as I could and all the bouncing on my belly was a really bad idea. So, start slow, and make sure you are good and healed before doing what I did. I am still paying for it. The pain?? I even had a fever all day yesterday, but none today, so I guess I am fine? -
It is currently 2 weeks after my Gastric Bypass in Jan. I know its just the beginning & its going to take time. But I do have a couple of questions. 1. I am having a bit difficulty drinking water. I didn't have any problems with water, I drank 6-8 cups of water, if not more. Now when I take even two sips of water my chest and back hurts, likes its full. I follow my doctor direction not to drink at least 30 minutes after eating. I am hardly getting 1 bottle in a day. I push thru the pain to drink an 16oz bottle. I was able to drink it more easier with crystal light mixed in which was only twice (or maybe it was perfect timing where everything was going smoothly internally). FYI, I drink from a bottle. 2. I was told by my doctor to start this past Sunday on puree food. I have such a hard time eating boiled eggs. I feel the same with like I do with the water even on an empty stomach. I chew it down to a mush. 3. I have pain on one incision (the main one). The pain is on and around the incision, not red and no pus discharge. But when I try to get off of bed it feels likes a big lump/ball inside that feels uncomfortable. Has anyone had these problems and what can I do to help? Are there any other problems I might face in the future? Thank You

I was at Dr. office yesterday and they say its not unheard of for some of us to still have GERD. Its just rare but if I dont take my PPI I burn just like before the bypass. I am still going to be scheduled for the bravo test. I cant wait for these non believing Drs to see my test results !!! That being said I have to stay on this medicine unless they can figure out why this is happening. I sit, stand , walk it doesnt matter I burn just like before just not all day. I am so happy for you !! 61 pounds lost is great. Please tell me you are feeling better too !! What is your goal weight?
