Revision from RNY to Distal RNY questions

I had RNY 21 years ago, I'm scheduled for a revision from proximal RNY (standard one most people get) to a Distal RNY where they make the roux limb longer by bypassing more small intestine than they do on the standard RNY. I'm getting this distal revision done Monday March 21st (6 more sleeps! Yikes!). My Dr says he does them as out patient, which kinda boggles my mind (I figured at least an overnight stay), and I'll go home same day. Has anyone else experienced this? I know there aren't many distal rivisioners that I've run across. -

I believe they meant Tore revision. I think autocorrect got them, lol. It's a revision for gastric bypass that is performed endoscopically. They make the pouch smaller by plicating and stapling a smaller pouch. -

if it turns out to be a stricture (about 5% of bypass patients get those), it's an easy fix. When they're down there, they'll just stretch it back out to where it's supposed to be. I had two of them (at four weeks out and again around eight weeks out). Painless, easy fix - and you'll feel 100% better afterward.

yea - most of us do. Chronic constipation is very common after sleeve and bypass. A lot of us take a capful of Miralax every day to keep on top of it. Been doing it myself for almost seven years now...

Let’s not pressure anyone into a surgery with risks. This isn’t “treatable” GERD. Many convert from a VSG to a bypass because the “gerd” is that bad. (A simple search here will show you what kind of GERD this is.) in fact, I don’t thing they should call it GERD at all. It’s like calling a “cluster headache” just a headache. I do agree with one thing - if obesity is killing you - get the surgery (the risk makes sense). If it’s not, then pass. For some weird reason, men aren’t experiencing this sort of GERD at the same rate or intensity - (Google 2022 study). Best of luck with whatever you choose. This is one time when you need to look at overall statistics- not individual experiences. Not even mine. If you’re OK with a 40% chance of horrible acid reflux (I never experienced it until after surgery) because your health is failing you - then I completely understand that. But it’s not a “pop a few tums” sort of GERD. Google long term PPI use.

Hi there. I had a revision from sleeve to mini bypass about 6 weeks ago. In that time, I've had quite a bit of diarrhoea and loose stools. Sorry for the TMI. It's really gross to me, too. My poop is green and smells UNHOLY. Is this going to last forever? I traveled to the Uk over the weekend and I was concerned about toilet access. Can anyone advise? Thank you.

I had gastric bypass on 2-22-22. I would like to say I'm extremely appreciative for everyone on here. The candid topics of bowel movements, pain in abdomen, food and calorie consumption, among other topics, have been refreshing. (Oh, and tips on how to edit a profile!) I do not really post but I am glad to have this community of peers to help and inform me through this change.

Thanks for sharing your experience! Glad you are on the mend. I will complete all of my pre op requirements at the end of the month so hope to get surgery in May. I will definitely be sure to be first. Still unsure about which type to get. I don’t have reflux and the sleeve is easier if I ever need a revision, but may loose more weight with bypass. Thanks all. While I did have gastric bypass in 2015, and pretty much aware of my issues with digesting some things, I just started MM last month, so...had no experience with this. The edibles are gummies, and I've tried both chewing them up and swallowing, or just keeping in my mouth as they dissolve and get the same results. At my next prescription renewal, I'm going to get all tincture, with no edibles. -
Bypass revision after sleeve? 8 years post op Sleeve

I converted from sleeve to bypass almost 2 weeks ago due to severe GERD. My recovery has not been typical. My small intestine swelled almost completely shut at one of the newly created junctions so I had to go back in the hospital for a few days for a lovely tube down my nose and into my stomach, more steroids and forced rest in the form of nothing to eat or drink. I'm fine now, fluids are moving through and my digestive system is fully back on line. The swelling wasn't a big surprise since my sleeve swelled quite a bit when I first had it done 7 years ago. Considering the severity of your GERD, absolutely get the conversion to bypass. The GI doc who did my first endoscopy was worried enough about the results of my second endoscopy (done by a different doc) that he called me to make sure I understood the severity of what could happen if it wasn't surgically corrected. I just wanted to update you on my experience. My surgery went very well on Tuesday and now on Sunday, I feel pretty much normal. My advice is to be first on the surgery schedule even if you have to wait an extra week or two. My surgery was moved up an hour and then delayed by 2 1/2 hours with little feedback and you can’t see your support person once they take you to pre-op. I was in pre-op about 4 hours. My surgery being so late also resulted in my not having a bed after recovery room. They put me in a cubicle in a very noisy area until I freaked out and spoke to the house supervisor. They finally moved me on the same unit into a real room. It was definitely not a post op unit, though. It was basically a do it yourself recovery overnight. Thank goodness I went home the next day. I’m certainly happy to be passed that part. Have you decided on sleeve versus bypass? Do you have any reflux issues right now? If you do, I would definitely just go for the bypass. I had the sleeve and ended up with constant feelings of indigestion plus chronic gastritis. Of course, it’s too early to know anything about my bypass experience. -

welcome to the club. Chronic constipation is pretty common after WLS - or at least sleeve and bypass. It's due to the high protein diet plus some of the supplements (namely, calcium and iron). A lot of us take a capful of Miralax every day. I've been doing it since I had surgery (almost) seven years ago. -
Band to Bypass Surgery coming up...SCARED

Hi whispers, I lost most of my weight with band (333 to 183) but then I gained it all back. When I started my weight loss pre- RNY I was back at 333. I'm now down around 160 and I lost it over a little over a year. What made bypass better for me: Food doesn't get stuck. I don't throw up. I don't have acid reflux (well, much. I'm one of the unfortunate ones that still has some). At 2+ years out I eat a "normal" amount --- normal for normal weight people, not my pre-surgery normal. That can be good or bad as if I eat the wrong things I will get too many calories. But it is better than with band. With band, if I was retaining water, or on my period I may only be able to eat a bite or two some days. Other days it felt like I could eat anything. For what it's worth, I also really embraced exercise and walk almost every day, hike multiple days a week, and hit the gym a few times a week now that they are open again. -

I'm prescribed MM for diabetic neuropathy pain, and got both tinctures and edibles and they do quell the pain and burning enough to allow me to have a restful sleep. Taking the same amount with both, the edibles are way less effective, so...I'm thinking I can no longer process them correctly due to my bariatric surgery? Does anyone have any experience with this or know that this is a fact? Thanks. -

To LilaNicole 20 ! Hope your procedure went well and you're feeling better now. My gastric bypass was on 3/7/22. I still have a little pain at the incision sites and a bit more intense pain on my right side so it hurts when i take a deep breath. My Doctor fixed a hiatal hernia he found during surgery, so that may be the reason for the pain. I can drink liquids easily, thank God so i won't get dehydrated, hopefully. Have to take PPIs a long time and still taking Metroprolol for high blood pressure, but may be able to discontinue those after a while. I can't walk very far yet or stand up straight without a pillow. Other than all that, i'm feeling really good! Thank You, Jesus!!! -
Weight loss with revision

I've noticed the weight loss is slower after the revision but still moving at a steady clip. I had my revision to bypass in Nov 2021 so still losing but I'm noticing more plateaus and slowdowns, it eventually starts moving again though so no concerns. -
After years of consideration I am going to gave gastric bypass surgery. I have a great doctor and will discuss this with her in a few weeks. I have very good insurance and the freedom to choose from a wide variety of surgeons. I am grateful. Is anyone from the San Diego area who can give a recommendation on a bariatric surgeon? I know my doctor will have info but I’d really love to hear from GP patients as well. Thanks

Bypass revision after sleeve? 8 years post op Sleeve

I was sleeved in 2015 and just went through the revision from sleeve to bypass about 3 1/2 weeks ago. So as I'm still in recovery mode I can't really provide a full answer all of your questions. 1. Reason for conversion: I developed a hiatal hernia where part of the sleeve had slipped up through the opening of the diaphragm into the chest cavity, which was causing chest discomfort and only allowing me to eat a couple ounces of food at a meal. After doing an EGD my surgeon was concerned enough to schedule my revision within a week. He said if they just fixed the hernia there was a high likelihood it would recur with the sleeve. If he converted to bypass he said there was a very low likelihood I'd have any further problems. I also had GERD issues from the sleeve which I wanted to address. 2. Post-op period: I'd say for me it was essentially the same. I had the same surgeon and facility which were both great. Recovery plan is the same for both surgeries (clear liquid, full liquid, puree, soft solids, etc.) I had a bit more pain with the bypass but I'm 7 years older than when I had the sleeve. 3. Best and worst part: Can't really answer this yet, except to say it's nice to lose some additional weight (I never hit my goal with the sleeve) but it's tough to have to start over with the recovery. 4. Dumping: Haven't experienced it yet, but I am just up to about 2 to 2.5 oz of soft protein so I have not been able to even try anything that might cause dumping. My thought on the revision is if you are experiencing significant GERD it is definitely worth considering. GERD can cause Barret's esophagus and ultimately lead to esophageal cancer if not kept in check. Having had a relative who died of that cancer in her early 50's I was concerned enough to want to solve it if I could. -

I was able to drive a few days after both surgeries (I had sleeve, then revision to bypass). I'm surprised they're sending you home the same day of the surgery however. Regardless, by Monday you should be OK to sit at a desk and answer the phone, just make sure you schedule some time every two hours to walk around a bit.
