I have anthem bcbs of ca mine took 5 days my surgeon didn't call to tell me I was approved I called the insurance company and asked them on the 8 day of waiting . she said you were approved 3 days ago I was mad no one called me but excited I was approved

I have done all my requirements my nut sent in her papers to surgeons on Monday I go tomorrow for surgery clearance testing at my pcp. Now my question who calls me to tell me I am approved and its a go? I just had the hospital call asking for my copay for the surgery. Well I caved again and over the weekend had 2 cigs. I'm doomed. My pre operative testing is Oct 6. My surgeon never said anything about smoking I did fill out the form that I was a casual smoker. But from reading everything online its better to quit before and I want to be in good shape for the surgery. Who is the one that makes the decisions about the patient being nicotine free? I have anthem bcbs of ca and I had to do 6 months with a nutritionist but was able to do all the other requirements while doing the 6 months. I have 1 appointment left with nut then I have my pre op testing a few days after that. I didn't loose any weight with in the 6 months, if any a pound or 2 those with my insurance have you heard them decline because you didn't loose weight? So I had to see my surgeon a second time yesterday no one really knew why I was there. He said maybe your insurance requires it. He never told me about a 2 week before surgery diet nor anything about shrinking my liver before surgery. Would that come from my PCP when I have to get the clearance for surgery?
