It mostly depends on your insurance companies requirements. I have BCBS LA , and they have NO REQUIREMENTS!! (GO ME) Just have to do the test that Surgeon requires. Psych evaluation (done) . You have the right to take FMLA, and they can't fire you. Not sure if your in an "AT WILL" state or not. Look and see what rules and requirements is in your state. Most states, you have to have been on that job for at least a year. The catch also is, they only have to guarantee you "A JOB," not necessarily the job that you had. Well Goodluck with that.
