Hi! It was easy to get approved, so long as you Dr. addresses each and every thing on that checklist. My Dr. did not on the first submission and I was denied. My Dr. resubmitted with the correct info being addressed and I was approved within 3 days. Good luck to you! Let me know if you have other questions.

I just saw your question about where he puts the ports. =) Mine is on the right side, below the 1/2 way point between my belly button and breast. The waist of my pants is below it, so I have no problems there. I was one of the first ones that he started putting it a little higher on. It is in a perfect spot!!
