    You have to go to your doctor for 6 months. They document everything from how much weight u gain or lose each time. Plus, they give you diet and exercise plans. I'm speaking from what I just had to go thru to get approved for insurance. It was a long list!! Fortunately, had been going to dr on a monthly basis complaining about my weight. I was a ahead of the game with that part. Good luck to you.
