My insurance also required this. I had to go to my primary doctor once a month, then my primary had to fax forms to my surgeon's office and then they sent those forms on to my insurance company.
I am trying to loose my final weight before my surgery and i need a guideline for a liquid diet. Does anyone have something I can go by? I am looking for something like a protein shake at breakfast so many ounces of broth at lunch and so many at dinner. Does anyone have any thoughts or guidelines to go by.
I am using Six Star Pro Nutrition, whey protein elite series triple chocolate. It has 30 grams of protein per scoop and 170 calories. I bought it at Walmart. It has a good taste.If you use skim milk or Almond milk it really punches up the flavor.
My doctor is requiring me to lose 14 pounds before I can have surgery. I have lost seven pounds so and my nurse says if I do not loose the other seven by the end of February I will have to do a liquid diet. Have any of you had to do a liquid diet before your surgery? If you did, what was it and do you have any suggestions on what to do to to loose the weight before surgery.
