Does anyone have BlueCrossBlueSheild-Lumenos Insurance?

Just curious if anyone has BlueCrossBlueSheild-Lumenos Insurance? If so, did you have any problems getting approved for the procedure? Did you have to do anything special? Pre-diets, programs, meet a certain criteria? Any inof would be super! I have my first appt next monday and I am very anxious! I just dont want to be let down when i get there to find out that i have to has certain things done or do certain things before they will approve.
