PCP recommendation letter---NEED HELP PLEASE!

Anybody here have any copies of letters of recommendation that WORKED from your PCP? I talked to my doctor today and he said he'd be glad to write me one and even said if you want to email me what you want it to say, go ahead and I'll put it in there.

Anybody have one? Or know of a site/place that I can get one?
