appt with dr

I'm going on thursday to see my primary care dr to see if I qualify. I am so nerovus about this. I guess I'm really embarrassed to go in there and tell him all my issues. My experience with military drs hasn't been that great though. They've always dismissed my probs. My husband is going with me. Is there any advise on just how to put things? or what to say?
