Medical History

So... I'm in my fifth month of my six month weight loss monitored program. Then, BAM, my surgeons office asks for 5 years of medical history. I never had health insurance growing up (father was self-employed) and don't have 5 years worth of medical history. Has anyone else had to provide this information?? More so curious than anything else. I have followed this thing to a T and now they spring this on me.

Yup, my ins co wanted that too and I didn't have it either ( its a long story why). I could only send a history from ob/gyn that had gaps. I was denied. Self Paid. End of story.

wow... that's actually what i am waiting on now.. They told me they were looking for records at least 1 from each year showing what my weight was. now I have the records from 2001,2002,2003,2004,2005,2006 they really dont care what you went in for just to show what your weight was.. Can you think about any where you might have gone that would have charted your weight at all?
