Stop Smoking?

Has anyone not quit smoking like your told, by your Doctors. I know some insurance companies want you to quit 2 months before surgery. Has anyone not stopped before surgery? Has anyone stopped and then started back up after surgery? Do they check your body for nicotine before surgery??
