Insurance question

Hello all! My doctor suggested bari surgery almost 2 years ago to which I declined..fast forward and I am ready. Has anyone been approved thru Anthem (BCBS KY) I have bmi 43.1 I have Rheumatoid Arthritis and Lupus, what was ur length of waiting if u had over BMI etc etc. Thank u in advance
