Pre approval weight loss program

I am just beginning this process. I met with a surgeon and looked into insurance coverage. The insurance company wants me to be under a "physician supervised" weight loss program for 90 days. The surgeon suggested to go to my primary care physician. I have an appointment set up but I am thinking based on previous visits over the years that she will not be supportive of me looking into surgery (BMI is 36 now). I am anticipating that she will just say to diet and exercise as I have done in the past. I want to get the 90 days set up ASAP. Any suggestions for where to go?
