Cost Estimate?

I'm wondering if the surgeon or insurance company provide a general out-of-pocket cost estimate prior to surgery. I know what percentage my insurance covers...but I'm curious how much I'll pay out of pocket.

    I actually got call this afternoon with what I owed before the surgery and it's $320 I have Anthem BCBS of KY
