On paper I have all 6 required physician visits but I noticed my doctor doesn't take the best notes . Do you think the insurance taken into consideration that that is not my fault?

Insurance is likely going to want good notes but your surgeon's office should be able to review the notes and know if you have a good shot or not. I remember my dietician told me I needed one primary doctor at the end of my three months of insurance required visits because the notes from the first visit weren't going to cut it. At the end of the day it's up to insurance but maybe the surgeon can at least give you their input.

Ugh. I've worked so hard for this to be denied because of a technicality. I'll be calling the insurance company to verify what they need and what I have. Thank you for your input.
