So today my surgeons office emailed me my requirements and they are:

1. A letter from my PCP which I have.

2. Blood work which I also already have.

3.?I need to attend one PreOp class.

4. I have to do a sleep study.

Than they'll submit it to my insurance and hopefully I'll be approved and they'll schedule my appointment. This sounds fantastic. The only downside is I work at a school and I'll have to take off from work or should I wait until Christmas break? That will give me two weeks off and I can take an extra week off, so that's 3 weeks combined

I taught this past year and had my surgery in February my surgery was on a Friday so I was going to take sick days for Thursday and Friday and then the following week. I got lucky though and we had a big snow storm and I only had to take 3 sick days. I was originally going have my surgery over Christmas break but my paperwork didn't get submitted for insurance approval as soon as I expected.
