Clearance and nutritional counseling

Does anyone know how long clearances last? I got my clearances over with such as heart Doctor, psychologist appointment, and nutritional counseling back in August 2015. Once my insurance got back to me, they said I needed two more nutritionist appointments. Friday is my last one.

I'm afraid I've gained weight. Is losing weight a MUST before surgery?

Also, does anyone know how long it to their insurance to have a final approval? I have WellCare if that helps.
