6mo diet-help!

So my insurance requires the 6mo diet. What exactly gets sent to the insurance for review? Since Jan this year I've been seeing my primary doctor about weight-loss. But it wasn't until August that I set my mind up for surgery because nothing has worked since January. Anyone think those past apts might count?

It should count. They want proof of 6 months (consecutive) medically supervised diet. Usually within a 12 month period.

If all the right information was documented and your doc is willing to piece it together and document it for the insurance co, it might work. I was given a form by my center to fill out on each visit for documentation. If you can piece together the history you might be in business, but there can't be any gaps! Good luck!
