surgery switch not welcome news

So for 3 months i have studied, researched and decided on getting a sleeve. After my egd results showed a small barrett...my surgeon is recommending bypass. The barrett is not of great concern but if it should become an issue, a sleeve would create difficulty in treating or following up on it.

I really dont want to get bypass. And im considering not even getting surgery if i cant get the sleeve. Has anyone run into this issue with barretts?

Did you have a biopsy?? Have you been treated for gastric reflux with ppi medication??

No biopsy (that im aware of)...he said the chances of it becoming cancerous is very low but im also young so if anything "should" go wrong...

He put me on reflux meds about a month ago.

How can he determine if you have Barretts Esophagus without a biopsy? I have had very bad reflux for years and was so afraid I would not be able to get the Sleeve but they determined it had not turned into Barretts Esophagus so I did get the sleeve. I would let your doctor know you want the sleeve and ask for a biopsy to see if it is just reflux or truly Barretts. What does the Gastro doctor say? Ask lots of questions....

Many vets here have Extreme GERD and other reflux symptoms. I have the sleeve and am fortunate to not experience any or rarely but I would be concerned if you already have some issues because research does show that the sleeve is not a good option for those people and can also increase it. I'm sure you don't want to hear this but I would be aware and weigh the choices very carefully.

Search the site for other threads. Good luck and let us know how it goes for you.

Without a biopsy, a Dx of Barrets is impossible. I have an irregular z line, but I don't have Barrets. This was determined by biopsies taken during my EGD. PPIs can manage GERD and prevent Barrets. Most VSG patients take PPIs the first 6 months while their stomach learns to make less acid and then wean off of them.

He's had to have done a biopsy then right? I was a little overwhelemed when he explained it so i may have missed a few details. I have a friend who had bypass who is fine now but for months after surgery she was in mad misery & im so afraid of having the same experience.

I didnt even realize i had reflux until after the egd. Thanks for being a straight shooter...your feedback is appreciated!

I had bad gerd prior to gastric bypass . When I talked to my surgeon he suggested the bypass because the sleeve tends to make acid reflux worse. I did go with my surgeons suggestion sense after my surgery I have not had reflux . I would consider the bypass . It will make you a heathy person .

Ppi = proton pump inhibitor, drugs like Crestor, prilosec, omeprazole to reduce acid production.

There are approximately 160,000 bariatric surgeries performed in the U.S. every year. If you personally knew 1,600 friends that had the same experience, that would be 1% of the U.S. surgery population for a single year. Consider re-visiting your logic. Do you really want to make a decision as important as this one based on one person's experience?

The majority of doctors do not recommend the sleeve for patients who have a history of GERD, even in the absence of Barrett's. You don't have to look any further than this forum to read stories from folks who have had the sleeve and are now having revisions to the bypass because of severe reflux issues. The sleeve is a highly successful, safe and effective option with a slightly lower complication rate than the bypass. But in your case, your doctor is making a medically based recommendation that the bypass would be a better choice. If you're not comfortable with that recommendation, then by all means seek a second opinion.

To walk away from this life-changing opportunity to live a happier, healthier and longer life, free of obesity, is a terribly high price to pay based on one friend's experience.

How right you are...i love this forum. The more i read here, the more sense your above statement makes. I am 4 months in...i cant turn back. Just scared of the unknown i think.

The doctor went on to tell me it's the shape of stomach that makes the sleeve a bad choice making Barrett's/Gerd/Acid reflux worse and with Barrett's lead to the next stage cancer. Before I left his office I was on board with the bypass. I know I need a tool and I certainly don't want the sleeve and risk getting cancer.

This site has helped me know I made the right decision hearing that they are fully recovered. My bypass surgery is set for May 7th, a week from today. :)

      Iʻm roughly 6 weeks post-op this morning and have begun to feel like a normal human, with a normal human body again. I started introducing solid foods and pill forms of medications/supplements a couple of weeks ago and it's really amazing to eat meals with my family again, despite the fact that my portions are so much smaller than theirs. 
      I live on the island of Oʻahu and spend a lot of time in the water- for exercise, for play,  and for spiritual & mental health. The day I had my month out appointment with my surgeon, I packed all my gear in my truck, anticipating his permission to get back in the ocean. The minute I walked out of that hospital I drove straight to the shore and got in that water. Hallelujah! My appointment was at 10 am. I didn't get home until after 5 pm. 
      I'm down 31 pounds since the day of surgery and 47 since my pre-op diet began, with that typical week long stall occurring at three weeks. I'm really starting to see some changes lately- some of my clothing is too big, some fits again. The most drastic changes I notice however are in my face. I've also noticed my endurance and flexibility increasing. I was really starting to be held up physically, and I'm so grateful that I'm seeing that turn around in such short order. 
      My general disposition lately is hopeful and motivated. The only thing that bugs me on a daily basis still is the way those supplements make my house smell. So stink! But I just bought a smell proof bag online that other people use to put their pot in. My house doesn't stink anymore.

      Oh yeah, something I wanted to rant about, a billing dispute that cropped up 3 months ago.
      Surgery was in August of 2023. A bill shows up for over $7,000 in January. WTF? I asks myself. I know that I jumped through all of the insurance hoops and verified this and triple checked that, as did the surgeon's office. All was set, and I paid all of the known costs before surgery.
      A looong story short, is that an assistant surgeon that was in the process of accepting money from my insurance company touched me while I was under anesthesia. That is what the bill was for. But hey, guess what? Some federal legislation was enacted last year to help patients out when they cannot consent to being touched by someone out of their insurance network. These types of bills fall under something called, "surprise billing," and you don't have to put up with it.

      I had to make a lot of phone calls to both the surgeon's office and the insurance company and explain my rights and what the maximum out of pocket costs were that I could be liable for. Also had to remind them that it isn't my place to be taking care of all of this and that I was going to escalate things if they could not play nice with one another.
      Quick ending is that I don't have to pay that $7,000+. Advocate, advocate, advocate for yourself no matter how long it takes and learn more about this law if you are ever hit with a surprise bill.

      Thank you everyone for your well wishes! I totally forgot I wrote an update here... I'm one week post op today. I gained 15 lbs in water weight overnight because they had to give me tons of fluids to bring my BP up after surgery! I stayed one night in the hospital. Everything has been fine except I seem to have picked up a bug while I was there and I've been running a low grade fever, coughing, and a sore throat. So I've been hydrating well and sleeping a ton. So far the Covid tests are negative.
      I haven't been able to advance my diet past purees. Everything I eat other than tofu makes me choke and feels like trying to swallow rocks. They warned me it would get worse before it gets better, so lets hope this is all normal. I have my follow up on Monday so we'll see. Living on shakes and soup again is not fun. I had enough of them the first time!! LOL

      Still purging all of the larger clothing. This morning, a shirt that I ADORED wearing ended up on top. Hard to let it go, but it was also hard to let go of those habits that also no longer serve my highest good. Onward and upward!
