How do we know if insurance covers surgery

A few years back I called our insurance and they called back a few days later and told me it wasn't covered. Now there is an online system and I pulled it up. Bariatrics isn't mentioned at all. Is there a Code that I should call and ask about to be certain?

I just called the insurance company and they told me where to find the coverage, precertifications, and what the benefits were. Do you have a customer service number on your insurance card?

I also think that any bariatric program/surgeon's office will verify your insurance for you as well. They know exactly how to do it.

I logged in to my insurance website and sent a message asking if bariatric surgery was covered.

The CPT code for laparoscopic sleeve gastrectomy is 43775. That's the code I gave to my insurance company to ask if they covered the sleeve gastrectomy.

We went to a WLS seminar and our surgeon's office found out everything from our insurance company.
