FMLA

Did anyone use FMLA to do their surgery? Were you required to have x amount of pto or pho before you were able to use it?

    Same here, I can use up to 12 weeks FMLA. The first week I have to put down as PTO, but I get it back when I return to work.

You will be much better served to ask HR in your company, as they will have the answers you need. All anyone on this forum can tell you is how it was in their company, which is of no use to you.
