Has any one ever been denied surgery by blue cross blue shield if so why. I ask because I will be submitting for approval soon and I want to be prepared

    Sometimes insurance companies will deny a request because the MD's practice does not have a "center of excellence" designation.

    That happened to me. I then switched MD's and was approved.
