January 17, 201313 yr Hi! Your best bet is to search for your medicare contractor that is doing the claims for your region. I looked up mine in Texas because it had just been approved in July by Medicare but took until December 10th to be approved by the regional contractor. If you find their website, look under they press releases. They usually have the procedure of how to bill and what diagnosis will be covered to get the sleeve. The. Print it out and give it to your drs office. Most drs offices are learning as well and might not know how to bill or get Pre approval just yet. If you wait on ten to do it you will be waiting a loooooooong time. I hope this helps and you get your surgery soon:).
