My surgeon required a pulmonology clearence, blood work, upper gi, chest X-ray, sleep study,and EKG. I noticed many of you needed an endoscopy, and a stress test in addition to these. I had a endoscopy in January 2012... Could this be why they didn't need another?

Probably, because sometimes insurance will not pay if you had one that recent. It's a little more invasive procedure than the others and quite expensive..especially if you are self pay. And since you had one as early as January should be all they need. Sometimes if a red flag goes up on other tests, the doctor will do further testing..that happened to me. Every insurance company is different, and then on top of that every surgeon is different...so mix up all the combos of the 2 it makes things different for every person.

That makes sense. Fortunately my insurance only needed info that my BMI was 40 or higher to receive pre-approval
