Dr. Kelly and financing

I have tried to contact Dr. Kelly's office online but the contact form seems to be invisible. Wondering if anyone has used medchoice for their financing and what kind of rates did you get? And if anyone has a phone number for Dr. Kelly?? Or a email address?
