Insurance denied skin removal even though I have horrible rashes lower and all around my belly botton. I plan on appealing this, any suggestions or has anyone else experienced this?

Is abdominoplasty covered by your insurance? If so, have your plastic surgeon submit with pictures of your rash and say that it is medically necessary to remove due to the recurring rashes.

Have your doctor document it, including prescribed treatments for it, for 6 months. Most will cover it after that time period has shown no relief with creams and pills.

Mis73 is right. You must appeal with pictures and documentation of rash, discomfort, infection, etc. all the the problems you've experienced. Moles or skin tags aren't covered either, but if they are irritated because your bra or waistband rubs against it then removal becomes medically necessary. The doctor needs to document medical necessity.
