denied by medicaid

I have a? Has Medicaid ever denied anybody I'm from San antpnio TX also do they always deny ppl right away lol I'm supposed to have surgery in Sept i just dnt have an exact date also waiting for my therapist to send a clearance letter to my advocate also how long does it take for Medicaid to get back with approval
