Getting "Fills" by a local doctor

I live 3 hours from my surgeon. Wish i could get my fills done closer to me. How do I go about getting that done. Anyone out there ever do this? How much does each fill cost? Currently my fills are "FREE" for 1 yr (I paid for them in a lump sum when i got my surgery ).
