It was closed yesterday, but I tried again today and I got to do the survey. I was almost all the way to the end and I hit the back button and it booted me so I had to start all over again. But hey, it was worth $50. WooHoo, I have a birthday in the beginning of Feb, that money with allow my band to buy me something nice!! ~Mandy

Just went through the survey. The selection criteria must have something to do with the timing of the lapband surgery. I had my surgery in Oct 2005. Try the survey again with a date of later this year. See if that works.
