I didn't get a date until a month before my surgery. That was after fulfilling a year of pre surgical requirements.

I went through Kaiser So CA San Diego, so it might be different there. But we didn't have to provide actual proof. They just wanted us to write it out with approx dates and weight lost. No receipts or other type of official documentation was required. Good luck!
