Thanks for the information, I have been seeing my current doctor since I moved to Texas 8 years ago (that's when the weight began to skyrocket out of control) So my weight is well documented, the problem was the diet history and Non Physician supervised weight loss sheet, wanted every diet I have been on Yikes!! I have been sitting here staring at the paper wondering where do I begin, and hoping the information I do remember is sufficient enough. My anxiety level has gotten better now. Thanks for the info every bit helps, I have been soooo stressed over this.
  2. This is like torture remembering diets, weight loss, weight gain, especially when I have been in a perpetual state of yo yo dieting for the last 8 years. I have my appointment with the surgeon on January 7th, I will let you know what he says about my diet history sheets. I will provide you with this information, hopefully it will help so you will have an easier time.

  4. I was wondering how detailed the diet history sheets have to be, I have well over 5 years of a BMI over 35 with 3 comorbidities. However with the diet history I can't remember the exact years and weight loss and gains from all of the Non-Physician supervised diets. I do have medical records from Physician Supervised weight loss however. Please can someone give me any advice on filling this out. I would really appreciate the help.
  5. You can find it at any pharmacy. Walgreens, CVS, Walmart. Children's Tylenol or Tylenol ES, they also come in generics. Just ask the pharmacist for help if you can't find it in the isle. Hope this helps.
  6. The test is an Arterial Blood Gas (ABG), it shows the amount of oxygen and carbon dioxide in your blood. This is necessary in order to test lung function, especially given the fact that your will be under general anesthesia.
  7. I too did not tell many people about this, just my family and 3 close friends. Good luck with your surgery!!
