Psych Test...whats Involved?
I did mine a few weeks ago and I felt exactly the same way.. Not too nervous, just an unknown. The doctor I went to relaxed me instantly. There was a written questionaire, very simple questions to I suppose gauge your relationship with food, your body image and if you have impulse issues, or issues with depression or addiction (other than food). Alot of the questions are repetitive but asked in a different way. The "interview" was a history of my weight issues as well as getting info on my support system. He stressed the importance of asking questions either to him/surgeon or using the boards. He felt these forums were essential to working through the process. Don't worry, its' pretty quick and painless.

Thanks for sharing that, I've been trying to focus more on my triggers and stopping them (best I can) before my surgery.

Tax Itemization
I read this article, http://www.cbsnews.com/stories/2004/03/01/health/main603233.shtml It does not stipulate that you had to pay for services in the US. I found this pretty interesting as I never even factored in being able to deduct some costs associated with WLS.

My First Dr Appointment
I felt the exact same way but was met with a very positive reaction. She was happy I was taking a big step to get my weight and health in general under control. I went into the appointment armed with lots of research and confident I was making the right decision for me, and in the end that is what is most important. Good Luck!!
