  10. Hey Everyone, So I have everything done to submit to ins. except the six months of doctor visits to update about my weight loss, eating and exercise habits. I am so worried that I will not get approved because my doctor has not been inputting the correct information in my chart notes for every month. I feel like my doc and his staff are a bunch of airheads who act like they never heard of wls insurance reauirements. So my question is, what is supposed to be included in the info for every visit?
