You have to be Amercican or is it international?

I'm very interested. I've always done well in responding to research. I'm 66 years old, African American, life time history of obesity, tried many weight loss programs over the years and now find myself weighing 280 pounds. I want to loose weight to live. I've been fortunate I'm not diabetic. Had back surgery four years ago. Having knee problems.
