Anyone ever had to take a urine or blood test to prove that they quit smoking? Anyone been denied for this?

One of the docs I was checking out, said they do a urine test to test for nicotine the day before surgery (I am self pay), and if you fail, the surgery is cancelled and all money is forfeited. According to them I had to be 'clean' for 8 weeks, and that was way too risky (on my bank account), so I passed on them.
