UHC requirements

I have UHC and I do know that they cover the lapband at 100% does anybody know what are some of the other requirements beside the BMI? I am so neverous that I might not have what it takes.:)

I think it all depends on what plan you have. I have UHC Choice Plus and it only covers 90% of the costs. You also have to use a facility that they chose. I found that out the hard way. :)

The other requirments were

  • you have to be 18 or older
  • have a BMI of 40 or higher or you can also have a BMI of 35 - 40 but you have to have 2 other comorbid contidions
  • have you use a docotor that's in the bariatric center of excellence

I had UHC they were good I had to be over weight for 5 years have proof, and go to a heart doctor. That was it. It is nice I got approved in about 2 to 3 weeks. I go in Friday. 2/29/08
