depends on your insurance policy. I don't know about yours in particular, but a lot of them have deductibles, co-pays, and co-insurance percentages you have to pay before meeting your out-of-pocket limit (after which the policy picks up everything). I'm pretty sure I'd met all those requirements before I had my surgery, due to all the testing I had to do beforehand (sleep study, nuclear stress test, EKG, bloodwork, etc), so I don't think I had to pay anything else once the surgery time came.

you can always call your insurance company and ask.
