Does anyone know if BCBSAZ PPO covers weight loss surgery? If so, please respond.

We can't answer that. Not all BC PPOs are the same. Your employer decides if they will include or exclude WLS. You need to look at your policy.

I had full coverage for surgery and fills with the BCBS Federal Employee plan. I'm not sure what the PPO offers, but check the booklet that you should be able to get from your employer or BCBS. Mine specifically said that it covered restrictive and combination procedures.
