Insurance timeframe

So my Insurance required me to have three months of consecutive weight loss and tomorrow is my final weigh-in. After that my understanding is that they will have 30 to 45 days to approve everything. In everyone else's experience, how long was it between Insurance approval and surgery being scheduled? I'm speaking as someone from the United States. My insurance is through Aetna and my surgery is being done by Saint Francis. I've lost weight every month, but I'm so afraid of being denied regardless.

I got my referral approved today! My endocrinologist actually approached me with this so I didn’t even have to bring it up. The lady at the surgery place said my metformin counts as weight loss program and I was on it for over 6 months so that is my “in” for that. No Jenny or neutrisystem bs. I am going to have my pcm send my records and I have even (recently) had a full heart Work up. So I now just need my psych evaluation and a couple of other things.
