Question regarding a fill

I would like to know that if you have a 4cc lapband do that means that

each time you get a fill, each cc is added together until the total equal

4cc. or do it means that each time you get a fill you can get up to

4cc?

Also can you have a protein shake with your meal and that can almost

be your liquid with your meal.

i have a 4cc band as well. the total amount of saline is 4cc. i went yesterday to get my first fill and my surgeon said the bigger you are, the bigger the band. he was going to inject 1cc and could not do it. why? i don't know. his pa is going to do it next week. kind of frustrating.
