Either way you can.

I was told by my nutritionist to cut back on the regular coffee, so I did. Cut my caffeine down to just one cup a day pre-op. Then, about a week after I had my surgery, I started drinking decaf.

I hear though, that some doctors don't even want you drinking decaf after surgery, so it's best that you check with your doctor or nutritionist. But decaf BEFORE surgery won't hurt anything.
