I'm trying to figure out how to apply for short term disability for after my surgery. J work for a small company no HR department..

I live in ny.. does anyone know how I can get this done. I'm so confused thanks in advance:)

I live in NY but I have a short term disability policy (I work for the state but we have policies we have to pay for to have that benefit). I'm not sure what benefits are provided if you don't carry a policy but you should check out the NYS Insurance Fund's website. Looks like there is where you would start.

Unum is usually the company most insurances go thru..go to their site n inquire..good luck on surgery

The company I work for is very small. I asked them a while ago they said I need to apply online.. it turns out they need to give me the paper work and I go for surgery Tuesday... thank you guys for your help:)
